Premenstrual dysphoric disorder (PMDD) is a severe and disabling form of
premenstrual syndrome (PMS). It affects bout 3–8% of women world-wide.
Disorder is characterized by a cluster of affective, behavioral and somatic symptoms that occur monthly during the luteal phase (after ovulation) of the menstrual cycle and lasting for about 6 days on an average.
Emotional symptoms irritability, depression, mood lability, anxiety, feelings of ‘loss of control’ and difficulty in concentrating.
Physical symptoms include
abdominal bloating, breast tenderness, headache, fatigue and generalized aches.
The disorder is treated with administration of Selective serotonin re-uptake inhibitors (SSRIs) like
Fluoxetine or Sertraline as the first-line medication.
Nonpharmacologic treatment include dietary supplementation of calcium and Vitamin B6.
Cognitive
Behavioral Therapy (CBT) has been shown to be effective in adjunct to treatment with SSRIs.
