Cardiologist 's  Response
Welcome to Health Care

It usually means disease of Coronary Artery - probably congenital in view of the age... / rarely other causes like valve or muscle disease
Consult a Cardiologist / get yourself examined / investigation like ECHO and Thallium stress treadmill - and/or Angiography are usually needed to diagnose properly...
Stop smoking – immediately, completely and permanently
