Thanks for your query and an elucidate history.
The reasons of the left lower testis are:
-Left testis is naturally at a lower than the right one due to anatomical variation and is normal in many people.
-Pathologically the left testis can be at a lower level due to some reasons like:
Varicocele which is more common at the left side.
Hydrocele.
Testicular tumors.
Epididymo-orchitis
Infantile inguinal hernia.
The pain may be associated in few of the things mentioned above.
The reason for the right testis to appear on a higher level may in fact be a normal position and appears higher related to the left one.
The pain can in fact be due to stretched or compressed nerves at its pathway due to strong muscular contraction one gets during
masturbation ot hernia.
I would suggest you the following::
-To get a clinical examination in lying down and standing position by a
General Surgeon or an
Urologist.
-Confirm with
Ultrasonography and Color Doppler examination
Once the diagnosis is made , be ready to follow the advise as per the Urologist.
Till then have proper scrotal support.
Avoid strenuous exercises or work.
