Why Are Small Patches On My Head Itchy?

Hi. My head has little patches on top where it feels scabby. Its not irritating unless i start to scratch at them. Ive also noticed hair loss as hairs fall out on a big scaley bit of skin with sometimes 3 - 4 hairs in that skin. Can you help me please?
