insulin gradually fails. and therefore the insulin produced is either not enough, or not effective enough.
Therefore the sugars reduce because the insulin in the body is effectively less. Since your medicine remained the same while your insulin reduced, the sugars rise again

kidney functions, just

renal failure, despite having normal creatinine
