Hello I Had Stage 1 Breast Cancer On The Lower

hello
I had stage 1 breast cancer on the lower medial aspect of left breast in 2009 at the age of 35years. Had bilateral mastectomy and diep flap reconstruction right away. No lymph node affected.No radiation, no chemo.
ER and PR +ve , grade 3, no her2
put me tamoxifen, took for a year, had to stop because of side effects...

2012 recurrence in the flap 3 masses all less than 1 cms. same hystology as first
received radaiation. no other treatment

Now pet scan shows one internal mammary node , suv 5.81 about 0.5 mm width and 1.8 cms length. no other sites

doctor suggests CT scan after a month, biopsy will cause lung collapse, if still suspicious ,
oopherectomy and aromatase inhibitor, no chemo and rad..

Can you suggest weather treatment on right track?
