Hello, Doc I Am 33 Years Old Having A History

hello, doc i am 33 years old having a history of Pulmonary embolism in 2011 since then was on acitrom 4 mg. till today but on 29 dec. 2016 at night 10 : 30 pm My BP & pulse got shooted 150/108 and pulse was 115. got admitted in hospital and get tested for the whole body including CT pulmonary angio and 2D echo & thrombophilia Profile. in 2D echo reorts showing mild PAH. PA systolic Pressure = 48 mmHg. mild LVH. Fair LV systolic function. no. regional LV wall motion abnormality. LVEF-53% good RV function. No pericardial effusion while CT Pulmonary angio showing " there are signs of late subacute/ Chronic pulmonary artery thrombosis involving the segmental and subsegmental pulmonary arteries in the lower lobes predominantly on right side.
currently, i am on beta blocker and anticoagulant therapy but no relief.
