If A Person Is Put On A Blood Thinner Due

if a person is put on a blood thinner due to dvt blood clots, but the blood thinner was thinning the blood too much and was put on vitamin k to thicken the blood back than the patient caught another blood clot that formed in the leg and traveled to the lungs the Dr put a chest tube that caused an infection that caused the kidneys to shut down and eventually caused death, could the death have been prevented by not taking the patient off the blood thinner completely but lowering the dose and not putting the patient on vitamin k?
