My Father Aged (80) Has Had A Large Hydrocele Swelling

my Father aged (80) has had a large hydrocele swelling of the testicles for some years he has been examined by Doctors in the past but refused medical treatment. The swelling leaks fluid and has become rather smelly. His personality appears to have changed as he becomes very aggressive when questioned as to why he's refuses treatment he does not appear to be in any pain. Can a hydrocele severe infection cause mood or personality issues.
