Brief Answer:
Note the changes....if any.

Detailed Answer:
Hi.
Thanks for your query, albeit very short and specific.
To recapitulate: Male/34 - have birthmarks spread over the body - have on testicles too - never done check on them - Wants to know whether dangerous to have on genitalia (testicles)....
Birthmarks are the areas of the skin where there is more pigment (that gives color to the skin) than the rest of the body. Being present since birth they rarely undergo changes.
They have no different characteristic on genitalia or anywhere on the body. The only difference on genitalia are:
Since they are covered most of the times are not easily under observation all the times as one can see all the time if present on face or exposed part of the body and hence can detect the changes early.
Secondly, since they are not exposed to sunlight, should undergo the least change.

Check for the following things:
- Any change in size, regularity of the edges or surface.
- Any itching or change in color.
- Anything unusual that has changed in them other than the points just mentioned.

If such changes are not there at all, there is no need to worry about.
But the slightest change should make you aware, take an opinion of a Dermatologist and go for biopsy.

I hope this answer helps you, please feel free to ask for relevant queries if you feel that there is a gap of communication.
