Based on your above mentioned data I think that these symptoms could be as a result of dermatological side effects of the lithium, lamotragine and effexia.
It is known that Lamotragine could cause as a side effects rash symptoms, including red, swollen, blistered, or peeling skin etc.
Lithium could cause as a side effects acne/acneform eruptions, chronic folliculitis, , papular skin disorders, generalized pruritus with/without rash etc.
Effexor cold cause as a side effects lichenoid dermatitis, furunculosis, pustular rash, vesiculobullous rash etc.
Furthermore I think you should speak with your doctor in order to change dose of these drugs or to stop them and as well to meet a dermatologist to take a prompt treatment.
