Burning Pain Near Scrotum, Numbness In Anus. History Of GERD. Is This IBS-like Episode Related To Scrotal Pain?
Last week, I felt the burning pain from my scrotum head all the way down to the side of my anus. Since then, I feel like I've gotten IBS and also lost some sensation in the anus. I say I think I had IBS because I've got a long history of stomach problems and have had similar symptoms briefly but I've had the cramping, bloating, etc for a full day. At first I didn't know what it was, and then I remembered the same symptoms so I began doing the BRAT diet for the remainder of the day and into yesterday. I able able to excrete waste and I do feel my muscles contracting when I push. But, I've lost that tightness where someone would be like "Uh oh, I better find a bathroom quick!" At times, I cannot feel my bowels emptying. It feels like the waste has "slipped through" even when I am "pushing" at a fair amount of pressure. Also, when I wipe my anus I barely feel the paper. One time, I bled from wiping myself too hard from a brief case of diarrea from stomach flu and the only time I felt the paper was when I applied it to the scraped area where bleeding had occurred. I should mention that I did go to a urologist and my GP while all this was occurring. The urologist said I was "fine" and "everything seems normal" after doing a visual and manual exam. I questioned him about testicular torsion and he completely ruled it out. He then fed me a line how "the scrotum can take a bullet" and how my pain is "nothing to worry about" - that was in July, just about 2 weeks after the incident. (This is the 2nd day of the new year and I totally beg to differ with him!). As I said, I saw my GP and basically he said to see my GI doctor when stomach pain occurs. The reason I'm questioning my stomach pain is, again, I've got a history of GI issues (GERD, Lactose Intolerance, and so on). I'm really wondering if this IBS-like episode is related to the pain down there or is it because of my eating habits and they just happen to be working "side by side" yet they haven't really "connected" since one existed before the other did. HELP!!!

I understand your concern and I am really sorry to learn the frustrating nature of your condition.
Your presentation does not look like the testicular torsion at all; be reassured on that part.
However, from your symptoms I feel that you had major trauma and you appear to have damaged your anal sphincter as well as perineal muscles. It is this damage that is leading to recurrent pain as well as the stool related issues.
I feel that your stomach related issues have no relation to the problem lower down and GI doctor shall be able to guide you better. He may probably also ask you to get your anal tone tested.
You can also consult a surgeon or a proctologist to assess the injury to the nerves supplying the perineal and anal region.

By the way, I have noticed that i get these nerves on the back of my shoulders twitching This usually runs down both sides of my back down to my lower back.
Could pudendal nerve entrapment be ruled out too?

It seems that you are having a reactivation of your irritable bowel syndrome and you would need to see your gastroenterologist for the same.
The pudendal nerve does not run in the area of your injury and no study needs to be done to rule out its entrapment.
I also wanted to ask whether you are a vegetarian or not?
