Answered by Dr. Sameer Kumar (36 minutes later)
Brief Answer:
Its a tocolytic and to be used under gyne supervision.

Detailed Answer:
Hello,
Thanks for the query to HCM,
ADILAN SR is isoxsuprine and is a safe medicine to be used in case of preterm labour if it starts before 37 weeks. The aim is to reduce intrauterine contractions and continue pregnancy till term, provided there has been no cervical dilatation or if the foetal growth is inadequate and there is an advantage in delaying delivery. This is to be used under strict supervision of your obstetrician and an attempt to curb down preterm labour warrants hospital admission. I hope i have answered your query in detail,
Wishing you good health,
Regards

Brief Answer:
you can stop this med.

Detailed Answer:
Hello,
At 37 weeks , the pregnancy is already at term and even if labour starts , if baby is adequately grown up as the ultrasound for foetal growth parameters would be able to tell, you can have normal labour, just that you have to be kept on regular monitoring during labour. AFI of 6.8 is not an indication of c-section unless the doppler studies of umbilical artery shows resistance flow or reversal of flow and if your pelvis is adequate.
As long as you continue adilan SR, labour may not occur, so i feel as you are already term, this med can be avoided under supervision of your gynecologist, so that labour may start anytime. Then you may be assessed for vaginal delivery as per your progress.

Detailed Answer:
Hello,
Labour may start anytime in next 1-2 weeks , hence stop adilan and you have to be on regular monitoring for AFI AND FOETAL GROWTH PARAMETERS. Induction of labour for decreased AFI 6.8 .is not indicated unless there is an IUGR or intrauterine growth retardation. Oligohydramnios is AFI less than 5cm.

Brief Answer:
possible cervical dystocia.

Detailed Answer:
Hello,
if the doctor tried with augmentation of labour with medicines and still you didn't have a cervical dilatation inspite of going into labour, then there are chances of cervical dystocia where the cervix doesn't dilate inspite of adequate uterine contractions. In that case eh next best option remains c-section.
