If there is a swelling that is soft and supple that can be pressed without any pain, this is most probably an external pile/'hemorrhoid. No antibiotic is needed.
If this is a small swelling that is fir, painful or tender, you may need an antibiotic but Azithromycin is not the drug of choice in such cases.
It is most important to know the exact cause of the swelling to have proper management as this can be a small abscess, boil, cyst as you have mentioned or a beginning of an abscess.
Per-rectal examination and proctoscopy preferably by a General Surgeon give the best chance to have a proper diagnosis.
