Question: Can calcium levels spike and then go down? From 14.2 MG/DL (ionized calcium 1.61 MMOL/L) to a normal range ? Also I have been taking a persceibed dose of vitamin d 50,000, I know this can cause a spike in calcium but would my vitamin d level be normal (vit d 25-hydroxy 34.37) if it was causing the spike. Finally, my pht was also normal (55.8 ph/mL) but I know these test cannot always show hyperparathyroidism. I am going in to see and endocrinologist, but until then this finding confused me. Also, I asked a similiar question last night and I am not answer shopping, but I am new to this forum and closed the answer out before asking these follow ups.

Calcium levels generally do not spike markedly and then revert to normal spontaneously.

The 50,000 IU dose of vitamin D can elevate calcium but yes typically it would also have to be high before it can elevate the calcium levels so significantly.

However, there are many causes of a high blood calcium. High vitamin D levels is just one cause. So if your vitamin D levels are normal, then it is imperative to investigate other causes of high calcium.

PTH values in the face of a high calcium are very important to know. There are several causes that need to be pursued based upon the PTH level.

For example, in primary hyperparathyroidism, PTH levels are high normal to high, while in vitamin D/calcium overdose, PTH levels tend to be on the lower side.

There are dozens of potential causes based upon the PTH result. The results of these blood tests can be quite complicated to interpret. It is not as straightforward as simply reading the numbers.

A qualified endocrinologist will elucidate the picture for you after appropriate evaluation
