Can Elevated Protein Level In Urine Cause Placental Abruption?
Question: Doctor,
First of all I want to thank you for your time reading this--I really did try and give you the “spark notes” version. I have a copy of my records available if that will help.
1999 (age 12) I was diagnosed with Nephritis—Kidney Disease. Had significant tissue damage (75% damage) was on Oral Chemotherapy, Prednisone, as well as a few rounds of Dialysis. I was not on dialysis long enough to have a permanent port. I was told that my Kidneys started regenerating and that I would not have to undergo Chemo, Dialysis, etc anymore. I was placed on a Blood Pressure medicine (Vasotec). I have pretty much been on this drug my entire life. While Pregnant with my son Ray I was put on Atenolol by my primary physician and then when I went to an OB They put me on Aldomet My blood pressure was controlled throughout my pregnancy with Ray. They had taken it just a few days before I had a placenta abruption at 30 weeks with Ray. Also, in the ambulance my blood pressure was normal. I was working from home, there was a tropical storm so I went to take a picture of the storm and I thought I peed in my pants---- there was blood everywhere. Ambulance said they I lost about 1 liter of blood. I called 911 at 4:05PM and Ray was born at 5:35PM. Reason for abruption is unknown. Emergency C section, under general anesthesia.
Currently, I became pregnant after 20mo after having Ray (Feb 10 is my conception date). Puts me at 18weeks pregnant (June 17,2015) Blood pressure is low—currently on no medication. An Ob did a 24 hour urinalysis and it came up with 960 protein and normal is 150 and they would like it at least under 300. That’s when they decided to drop me. No other OB group has agreed to take me that I have tried. I want to go to Florida Prenatal (high risk dr’s) bc Dr. XXXXXXX has been mentioned to us by 2 different people. I have to have an OB refer me to Florida Prenatal and they only OB that’s taking me is USF Health. USF Health has their own High Risk Dr’s.
Problems with USF Health—the hospital they are affiliated with is far away from my house. If Placenta Abruption happens again- I am scared I will loose the baby—side not I might loose a lot of blood too. Also, first appointment I had with them I had to insist on a urine sample—seemed weird that part of the reason I was there was due to protein in my urine?
Regular ultrasound scheduled for XXXXXXX 23, 2015—right now no scheduled Dr consult. We are going to try and speak with someone while we are there. My next consult is July 13, 2015.
Thank you so much for your time. Please help me with any advice, knowledge, do's and don'ts, supplements, foods, excersize etc
Sincerely, XXXX

1. What is your serum creatinine level? When was it done?
I do not see Serum Creatine level listed in my records. These are the highlighted "red" or above reference range.
From 24 hr urine sample taken 5/5/2015:
Creatinine Clearance 166 mL/min (Range 75-115 mL/min)
Protein /Creatinine Ratio 492 mg/ g creat (Range <OR = 84mg/g creat)
Protein Total 24 hr ur 960mg/24 h (Range <150mg/24h)
What does this say about my kidney function? What stage kidney disease?
In 1999 was diagnosed with "Pauci Immune Rapidly Progressive Glomelunar Nephritis" I have had no relapses since 1999.
2. What is your current blood pressure reading? When were you taken off the meds?
According to records:
4/30/15 was 118/62
5/5/15 was 110/80
5/28/15 was 100/60
I have had it done at dr's after 5/28 but I cant remember what it was but it was "normal".
I did it at my house yesterday at it was 127/82
I stopped taking blood pressure medicine a few months before I became pregnant-- I didn't have a refill, I know its terrible.
3. Are you on any medications right now? Please list them out to me
I am not currently taking any medication right now besides my 1 a day prenatal vitamin.

Hello
Thanks for getting back to me, I am sorry for the short delay in my reply.
Here is how you should go about it
1. The parameters that have to be checked are serum creatinine; 24 hour urine protein and creatinine. The reports you submitted do not have these details.
2. Please make sure that your blood pressure is under good control, this is the single most important entity
3. Any rise in creatinine should be dealt by a nephrologist
4. Please have a low salt diet. Food with excess salt like chips, popcorn, Etc should be avoided
5. Do not engage in strenuous physical activities
6. Buy a blood pressure monitoring machine and check blood pressure regularly at home.
Serum creatinine more than 2.5 mg/dl and proteinuria more than 1g/day are considered as a bad prognostic marker.
I don't think logistically I can help you refer to an obstetrician physically. If you wish for personalised online help with your obstetric concerns, you can seek OBGYN specialist help from this portal.
