I have read your query and understand the concern.
You are having mild to moderate hepatomegaly counting your liver span 18.5 as stated by you.
If there are no lesions in the liver on your ultrasound,it could be either simple enlargement or it could be due to infiltration of the liver with fat cells (as fatty liver) which can be secondary to alcohol abuse or non-alcholic liver disease but I will give my opinion after watching your whole ultrasound scan report.So,it would be helpful for me if you can provide me the same.
Liver enlargement could also occur with other liver conditions, such as hepatitis but in my opinion, it is least likely to be the possibility here as you had all liver function tests normal.
Can you provide me some more reports like blood counts or viral markers of liver (HBSAg) if you have done it earlier which will also be helpful to rule out the possibility of hepatitis.
Gallstones do not lead to liver enlargement (hepatomegaly).
In your case,as your gall bladder is swelled-up due to gall stone then laproscopic choleycystectomy is advisable.

Hi,
Thanks for the follow-up.
It is good that your hepatitis report has come negative as i was expecting.
As i said earlier,if your gall bladder is swollen or distended and the wall has become thickened then it best to undergo it's removal and send the removed gall bladder part for histo-pathological examination as a part of routine procedure.
Nowadays, laproscopic choleycystectomy is the gold standard treatment in symptomatic cases gall bladder stone or infection or distension.
So,It would be better to undergo surgery within 24 to 48 hours to avoid any complications from stones or distended gall bladder.
Also,It is not advisable to wait for pain management and then to go for operation.However, your treating surgeon may have some different opinion.
Lastly,yes there is more chances of metaplastic changes in gall bladder wall due to gall stone or its infection.
Therefore, for that reason also,you need to get operated on urgent basis.
Means,sooner or later they are going to operate it so,waiting for just pain settlement is not justifiable.
You can discuss this things to your treating doctor and can act further.
