Child Having Hives, Fever, Red And Itching Ear. What Is Causing This?

Question: 23 month old broke out in hives and fever few days ago finally got fever to break yesterday but now she's waking up in the middle of the night crying and grabbing behind both ears that seem to be red and itching irritated what could this be? The hives have not returned since her fever broke yesterday

The symptoms you mentioned most probably resemble that of a viral infection likely to cause rash , fever, and congestion ( ear , throat pain etc) .
Another possibility is that of an allergic reaction to any food or medication or environmental allergen but in that case it doesn't last so long.
Most viral infections subside on their own within a couple of weeks but might require supportive medications like acetaminophen (10mg/kg/dose)for fever and chlorpheniramine (2mg/ 5ml)for itching rash etc.
The fact that there is itching and redness of her ear makes it likely that her eardrum is congested and painful causing the discomfort . As mentioned above analgesics and antihistamine medicines can decrease her discomfort but a thorough ear examination by a doctor is necessary to confirm the diagnosis and rule out any significant complications like bacterial superinfection or pus formation.
It's best to take her to an urgent care facility if the pain , itching persist despite such measures.
