Diagnosed For Meningtis. Had Fever, High BP And Paralytic Attack. Motor Skiils Are Not Good. Suggest?
Question: My mother-in-law (Shakuntala Shrivastava, F, 56 years) was brought into emergency ward last Monday mornig (6th May) with high-fever, high-BP and high-Sugar levels. Based on Spinal culture and CT scan, She was diagnosed for Meningitis and treated started accordingly. MRI report says the following: Bilateral Water Shed Infract and Chronic Ischemic changes in Peri and Para Ventricular White Matter. She had a mild paralytic attack in the process, and her right hand motor skills are not in good shape.She is currently in BHEL Kasturba Hospital Bhopal, and Dr S.K. XXXXXXX (Neurologist) was consulted. We will be shifting her to Hyderabad in a week's time and are looking for a Neurologist to continue the treatment at Hyderabad. Please advise us on the MRI diagnosis and treatment options at Hyd.

Your mother in law has two separate neurological problems:
1. Meningitis, also called brain fever. It is often caused by bacterial infection, and in some cases TB infection. The diagnosis is based on spinal tap. Treatment is with antibiotics and other medications.
2. Brain strokes- this results in paralytic attack. In fact, infarct and ischemic changes as mentioned on the MRI report are suggestive of brain stroke. The usual risk factors for this are high BP, high sugar and cholesterol.
Again, treatment is with medications and physiotherapy. Risk factors also need to be treated.
At Hyderabad, I work at Apollo Hospital, Jubilee Hills branch. We have good facilities for treating neuro patients. We would be pleased to take care of your mother in law.
