Question: I am a diabetic (type 2) and have been able to control blood sugar with metformin until recently. My last a1c was 7.3 and Dr added glimeperide. Sugars have been ok until the last week, and have run around 200-220. I developed pain in the kidney area, and wondering if the higher levels are causing the pain from kidney damage. I do have a history of kidney stones, but the pain level now is around 6-7 out of 10, and when I had stones, it was much greater. No other symptoms....

Thanks for writing to HCM.

The increase in blood glucose will not contribute to pain in the loin area. The blood sugars are definitely on the higher side. With a setting of loin pain and a prior history of renal stones that still remains a probable cause. The pain due to renal stones can vary in intensity depending on how large the stones are and the position of the stone.

I would advise you to do an ultrasound of the abdomen and a urine routine examination.

If you belong to the normal/over weight / obese category of weight then medications from the class of DPP-4 like sitagliptin, vidaglpitin etc would have been a good choice to add to metformin instead of glimepride because they target after meal sugars very well (which is the main target to decrease a1c further where it is below 8). Not that glimepride is bad but it can cause weight gain and hypoglycemia (too low sugars).
