Brief Answer:
Pain is a possible cause

Detailed Answer:
Thanks for asking on Health care magic.

Excruciating pain can indeed cause the heart rate to jump to 250/min. The other possibility is a phenomenon called paroxysmal supra ventricular tachycardia. You need not worry. Unless very uncomfortable, it does not require any intervention.

In any case it requires clinical correlation. The ECG tracing needs to be checked and it needs to be correlated what you were doing at the point when the heart rate was 250 and whether you were having symptoms.
