Had A Blood Donation And The Report Showed Positive HBsAg Aand HBC. Do I Have Hepatitis B?

Question: I had a blood donation on 11/06/2013.And the report is saying that:
HSBAG:Reactive
HBC:Reactive

Can you please suggest how much chance do It show for Hep. B?
And any suggestion for further treatment?

Dear XXXXX,

A positive HBsAg and anti HBc means that you have hepatitis B infection.

Now there are three stages of this infection 1) completely suppressed infection, 2) active infection but no damage to liver and 3) active infection and damage to liver.

Based on the stage you have further management can be formulated.

You need to do following tests before we can discuss this further.
1. LFT
2. Haemogram
3. HBeAg
4. anti HBe Ab
5. HBV DNA PCR Quantitative
6. USG liver

I presume blood bank must have already screened you for HCV and HIV infection.

Also your family (parents, wife, brothers and sisters, children)needs to screen for hepatitis B infection by doing HBsAg, because one of them may also be positive.

I Had a secondary consultation with Dr.J.S.Bhogal from Fortis hospital Noida. I just ask for further test and consultation. He just ask me for LFT and Ultrasound Whole Abdomen.

Here is some report detail please check and suggest if any further test or consultation is required as he suggested me to get another check-up after 1 month for hepatitis. And then after if anything comes up then he will suggest for medicine if required.

LFT Detail:
Total bilirubin:0.5 mg/dl
Bilirubin Direct:0.1 mg/dl
Total Protein 6.7 g/dl
Albumin: 3.8g/dl
Globulin: 2.9G/dl
Albumin/Globulin ratio: 1.3
SGOT :49 U/L
SGPT :96 U/L
Alkaline Phosphatase : 121 U/L
G-Glutamyl Transfer : 43 U/L
Lactate Dehydrogenase :179 U/L

US Whole Abdomen

*Liver:is normal in size and shows presence of increased echogenicity.No evidence of Focal Masslesion is seen.The intrahepatic biliary radicles are not dilated.Portal vein is of normal calibre.

*GALL Bladder: is normallydistended. No evidence of any calculi or mass is seen.The gall bladder wall thickness is within normal limit. CBD is normal in appearance.

*Pancreas: is normal in size , shape and reflectivity. No evidence of any focal lesion or peri-pancreatic collection is seen.

*Spleen : is normal in size ,shape and reflectivity. It measure cm.

*Kidneys :Both the kidneys are normal in size and shape. They show normal Parenchymal reactivity and CM differentiation. No evidence of any hydronephrosis / Calculus disease/mass lesion is seen.
Right Kidney measure: 9.1 x 4.8(in cm)
Left Kidney measure: 9.4 x 4.6(in cm)

No ascitis seen. No nodal enlargement noted.
Bladder: is normally distended and appear normal.
Prostate: is normal in size and measure 13.4 cc in volume/weight (in grams)

Impression: Grade 1 fatty liver.

Please suggest if any further test is required.

Thanks for contacting again.

Your present reports just show that your SGOT and SGPT are raised and that you have fatty liver.

I have already suggested that you need a battery of tests as enumerated by me earlier.

These are as follows, you have already done LFT and USG, so please get the other tests given below and then we can discuss further.

1. IgM anti HBc antibodies
2. Haemogram
3. HBeAg
4. anti HBe Ab
5. HBV DNA PCR Quantitative
6. USG liver

Is there any difference b/w USG whole Abdomen and USG Liver.
If so then can I have to go for USG Liver again?
