Severe pain in buttock and coccyx is unlikely related to diverticulitis or perforation you have had in the past.
Possibly it is related to some spinal problem or local perianal problem like perianal abscess or rarely pelvic inflammatory disease. Spine problems are usually related to posture and some time be sharp shooting types. Perianal abscess presents with pain while defecation and pus discharge in addition. Pelvic inflammatory disease can present with vaginal discharge, painful coitus or dysmenorrhea.
Another possibility is coccydynia which is associated with inflammation in coccyx causing pain.
An MRI of spine and pelvis will help sort out the issue.
