2. Do you exercise? If yes what exercises and how many times a week
3. Do you consume alcohol? If yes, How much in a week?
4. Was any ultra sound scan of the abdomen done? If yes please let me know the report
5. Please mention all the medications you are currently on
6. Did you approach any doctor with these reports? If yes please mention the prescribed drugs
