Question: hi doctor i have problem on both my thumb fingers. Just below the nail i have severe itching and my skin is getting dark on that area. often i am getting this problem and my skin on that particular area is peeling. Please give me some tips to cure this.

Thanks for writing to us.
You seem to be having eczema of your finger tips. This could happen due to exposure to harsh soaps, detergents, sometimes due to cutting and chopping of vegetables etc.

To treat the same
Apply Clobetasole cream twice in a day over affected area and once the area heals completely you may stop its application. This will help not only in peeling of skin but also reduce darkness.
Moisturise your hands several times in a day with cotaryl cream or moisturex cream. To prevent further episodes also, continue with these creams.
If there is too much of itching, you may take tablet cetirizine twice in a day
Use very mild soap or detergents to wash hands or clothes
Wear gloves while doing any house hold chores

To wash hands- you can use cetaphil ,pears, elovera, hidrate or venusia soap bar
To wash clothes- you may use ezee liquid detergent

Hi doctor earlier i had white patches on my chest and back. For that i had kz cream and used novale soap. Now it got cleared. Now i am pregnant 7 months. U have suggested me clobetasole cream for my thumb finger problem. Is that safe to use it now? and also i have hard, black and rough back(seat). How to make it soft and make it normal like my other skin? And please suggest me the soap for daily use which will never give me white patches in future mainly during summer season. does dandruff related to this?

Hi,
Steroids can be safely used in pregnancy without side effects. However, to be on the safer side, I would change your prescription for eczema
Use a moderate potency steroid cream like mometasone cream and apply it only over the areas involved. Once the fissuring or the cuts heal, discontinue its use. It would take five to seven days for the lesions to heal. Clobetasole cream too can be used but as its a potent steroid and it is difficult to maintain follow up online, I would suggest a milder steroid cream
Instead of using cotaryl or moisturex use Secalia cream twice in a day. It is completely safe to use
Take tablet cetirizine for itching only if required otherwise can avoid. Cetirizine is safe in pregnancy.

As far as dry hard rough skin is concerned, in pregnancy use Secalia cream daily over the area to soften it. To some extent when roughness will decrease, pigmentation will also decrease. Later on once your pregnancy is over and you have stopped breast feeding, you may start applying Xerina cream, its a keratolytic cream that removes dry dead cells along with pigmentation(as contains Glycolic acid) and keeps the area well moisturised.

Your white patch problem is pityriasis versicolor. Sometimes it is recurrent
To prevent its recurrence
Wear loose cotton clothes
Avoid warm humid climate and sweating
You may use candid soap for bathing. It's safe in pregnancy and lactation also.

Dandruff may occur with these white patches as causative agent a fungus is same for both. You can use nizoral shampoo to control scalp dandruff in pregnancy, however its large surface area application like over whole body as in pityriasis versicolor is not advised in pregnancy.

Thanks a lot for your detailed answer. I am happy with your answer. Now i dont have pityriasis versicolor. I had it before my pregnancy. So i mentioned you that i had it before. Does hair removal affect skin? I use to do waxing at times during functions and all and i use XXXXXXX french also at times. Which method is safe? Does XXXXXXX french is safe to use? I would like to have silky skin and hair. But because of my problems i worry a lot :( :( You are looking soo beautifull. What should i do to have such a skin. Ocassionally i use fair and lovely. Is it good to use fairness cream? It is good to consult a doctor regarding this so i feel free to ask you this. Please suggest me the right cosmetics and right foods for problem free skin.

The precautionary measures I have suggested is to prevent any further recurrence of P versicolor in summers

Hair removal does not affect skin. Waxing is better than using depilatory creams as results are long lasting because hair is pulled from roots and with depilatory creams there are chances of developing pigmentation or skin irritation. Prior to waxing, make it a point to exfoliate skin as it prevents ingrowth of hair.

To have good skin follow cleansing toning and moisturising schedule twice in a day.
Use a mild cleanser, followed by toner( do not use in winters) and a moisturiser. Always use a sunscreen in morning. In the night use a good hydrating cream preferably containing vitamin A, C, E. Exfoliate, steam and apply a face mask of your choice once in a week. Eat healthy nutritious food , salads, nuts and fruits daily.

Use a good moisturising soap to bathe and apply a moisturiser immediately after bath. Take warm oil body massage once a week and exfoliate body once in a week.
Similarly massage your scalp with warm olive oil weekly. Use a good shampoo, conditioner to wash hair and use a hair masque suitable to hair type once in a week.

To have a good skin, you have to be regular with your skin care regime and take a good diet . Always use products suitable to your skin type. Brands and cost do not guarantee best results.

Thanks doctor. There are many chemical products in the market. Among those products which one i should use? Suggest me some good cleanser toner moisturizer along with shampoo and conditioner. Is secalia is a moisturizing cream? Can i use tat as a moisturizer? And also can i use these medicated saop bars candid, cetaphil throught or only during the problamatic condition.

Hi,
Neutrogena, cetaphil, Clinique, sebamed, La rochey posay, Olay offer great products for skin. You can choose products as per your skin type
Shampoo and conditioner- Loreal, Wella, Schwarzkopf, dove
Body moisturiser- you can apply Secalia, venusia or cetaphil restoraderm.

Candid soap is only required if you have history of recurrent p versicolor. If there is a history you can use it in season of recurrence. Otherwise use mild soap as I have mentioned in my previous post( cetaphil, hidrate, venusia, dove pears).

Basically products have to be chosen as per your skin type and requirement.
