Question: I am a north indian and currently staying at chennai where climate is too hot for me. I used to have dietary issues, if i eat anything made of wheat then i used to have boils on any part of body, especially vagina and groin region.i had to change three doctors to get the root cause diagnosed.when m in north, i can eat anything, and m absolutely fine there.the diet solution that was prescribed by doctor was to eat rice and cool and juicy foods like cucumber etc.I am not supposed to even have curd but buttermilk.now i am pregnant for four to five weeks.I met the gynaecologist last week and she has prescribed the medicines like Sinate OD,F Safe, Dubagest 200.Are these tablets okay to be taken ? Moreover she has asked to get a scan done (ultrasound) next week.Is that okay as i believe that first scan is done after 12 weeks of pregnancy.

Hello XXXXXXX
Thanks for your query.
Please provide me the details of your last menstrual period.
That would help me to establish weeks of gestation, so that I can guide you regarding the ultrasound scan.
Sinate OD needs to be taken if you have nausea and vomiting sensations in the morning or through the day.
You can skip taking F safe if you are taking Sinate OD as they are for the same indication.
Dubagest contains progesterone, it is given to women with a high risk pregnancy such as previous miscarriage, conception after long period of infertility, spotting and cramping in early pregnancy, high risk of abortion in current pregnancy etc.
If your pregnancy is normal in every sense, without any of these risk factors, I see no need to take Dubagest also.
You would have to be very particular about your diet given your history of boils due to heat.
Eat simple home made foods and take lots of fluids - buttermilk, chilled milk, soups , kanji water etc.
Get your blood sugars tested as you have a tendency for boils.
Looking forward to hearing from soon with more details.
All the best.

My last menstrual period was on 9th August 2013 and usual period duration for me is 5-7 days (for getting it fully over), hence please suggest regarding scan.
there was spotting(reddish brown, grey as well as bit blackish in color for say,6-7 days ) in the early days of pregnancy, now only white discharge goes. is that infection or is that okay ?

Detailed Answer:
Hi XXXXXXX
You are currently 6 - 7 weeks pregnant.
The first ultrasound scan should be done in the first trimester , the earlier the better, as it gives exact idea of the gestational age of the fetus.
By 6 weeks, cardiac activity ( heartbeat ) is seen in the fetus.
In your case, as you had mild spotting also, it is more important to confirm that the baby is alright, and the pregnancy is progressing normally.
Your doctor is absolutely right in scheduling an ultrasound scan for you.
Please get it done, and ensure that it is done transvaginally by a qualified person, as abdominal scans in first trimester are far less reliable.
As you had spotting in early pregnancy, your doctor is justified in prescribing progesterone to you also.
So please take Sinate OD and Dubagest ( if you have morning sickness also ) or you can take just plain folic acid and Dubagest.
Also, as you are sensitive to most foods and medications, Dubagest can be taken vaginally, as it would be tolerated more easily than oral Dubagest.
Take care.
Please feel free to discuss further.

I got pregnant before too, within 3 months of my marriage i.e. in 2011(i am married for two years out of which i was away from my partner for almost 10-11 months).
The spotting was not there in my earlier pregnancy and the test reports and ultrasound scan reports were found to be absolutely fine. When the first scan was done it was 5-6 weeks pregnancy.Since it was bit surprising and unexpected hence as per the discussion with my husband and gynaecologist that time, we planned not to have an issue and hence the doctor aborted the same by use of tablets (orally and vaginally).

This time it is planned pregnancy and the last menstrual period started on 9th Aug 2013,hence the ovulation must have been around 22nd Aug till 25th Aug,please suggest. now there is no spotting and only white very fine discharge,
sometimes vaginal itching, that too is not nowadays.

Moreover, I am okay with the reply as per your suggestion, taking care of my diet too.Though eating rice, eliminating the starch too, I have put on weight.
Earlier my weight was 52-54 kg now it is close to 58 kg.

The gynaecologist I met last weekend has also suggested me not to have intercourse with my partner for next two months, no long distance travel, no weight lifts, and eat whatever I like.I can lie down in any position while sleeping.I told her about me sweating sometimes in AC too (may be due to low BP), she checked my BP and it was found to be normal. She told in Chennai, everyone feels hot hence no need to worry. Moreover, we did not go there by any reference hence not very comfortable while talking to her as she took everything very casually and did normal checkup and asked if i had any kind of abdominal pain. She was in hurry as as you know we north indians have language problem, however she talked to me in English but she was asking the questions very fast and asked us to come next week with scan report. The medicines she mentioned were only available at specific medical store that she mentioned in her prescription and none other shops keep these medicines.

When I eat cornflakes and no wheat only rice, then too sometimes my stool is tight (only in the beginning, later it gets passed smoothly) while passing the same in the morning. Moreover, should I eat pomegranate or not as someone asked me not to eat papaya, pomegranate, guava etc.

With boils earlier I had this stool passing issue too that time (two yrs back),it was pathetic experience and i used to cry in the toilet due to severe pain at the anus and anal outer edge and used to pass stool by not putting much pressure , only little bit contraction-relaxation, then when the third doctor suggested not to have wheat/ roti/ chapati/ parantha when i am here at chennai, i took only rice; sometimes i used to have roti, paranthas (at least twice a week) as i get bored of rice, i don't like it at all.Now there are no boils or any such issue. Earlier two doctors told that it was due to fungal infection during sex ; we changed the drinking water also as we order the water cans from outside everyday.

Should i do some yoga /exercise or not as it is 6-7 weeks pregnancy ?

Detailed Answer:
1. Regarding your spotting, only after an ultrasound can it be told if the baby is alright and the pregnancy is normal.
2. White discharge is alright as it is common in pregnancy, as long as you do not have excessive itching / bad odour in discharge / crampy abdominal pain.
3. Weight gain during pregnancy is common, yours is slightly on the higher side. As your BP is fine, do not worry. Try to eat small frequent meals instead of binge eating at a time. Cut back on rice, potatoes, ghee, oil , sugar, excessive salt.
4. Medications prescribed by your gynecologist are fine and known brands, so do not worry. Avoid stress, intercourse, travelling etc. till your ultrasound result is known.
5. For constipation, take lots of water during the day . You can eat any fruits, as fresh fruits are good for you.
6. Avoid any kind of yoga or exercise till the ultrasound tells you that the baby is normal.
Take care
