Hello and welcome to HealthcareMagic..
I thank you for posting your query here.
I will analyze all your points and provide necessary explanation below..
1. Age 75
Swelling of 1 x 2
Hard

OK, this tells me that she's having inguinal lymphadenopathy.
And since it's hard and also considering her age we have to be little aggressive in the work up and further management.
Please answer these questions..
A. Did she suffer or get treatment for any kind of cancer?
B. Is she diabetic?
C. Any ulcers in the lower limbs?
D. Any other symptoms?
Please let me know these and I will guide you further on how to go about..
Take care..

She has history of cancer. She is not diabetic nor does she have any ulcers or other symptoms.
Brief Answer:
What cancer?
Detailed Answer:
Hello and welcome back..
Please provide details of what cancer was it, when was it diagnosed, what treatment was given..
This will sum up everything..
Mostly, I feel it's a metastatic lymph node unless otherwise proved.
Best is to get a FNAC of that swelling..
Please get back with the details..

Isn't it true that there is a high % of false positives from FNAC's. (I assume that is a needle biopsy.) What is the best and accurate biopsy?
Brief Answer:
Fnac first
Detailed Answer:
Hello, welcome back..
Yes, definitely biopsy is better but its invasive and little morbid compared to fnac.
If there's something abnormal on fnac then next step is biopsy.
So please get fnac first..
Hope this helps you..
