Proton beam radiation therapy is new modality of radiation therapy where diagnosis of cancer of the prostate is by doing Prostate Biopsy where radiation is precisely targeted to kill the cancer cells without damaging surrounding tissue.

It is a new modality practiced in very few centers in USA and long term results of this modality are still to be documented ..

what if i do nothing

Doing a PSA screen is deeply personal as there are no clear benefits from screening and treating for prostate cancer in elderly men. If I were you, I will not follow up on this as it may not provide any huge added advantage over doing nothing. At times, it rather makes live more terrible. There are also several other causes of increased levels of PSA that have nothing to do with prostate disease.

Most persons with cancer of the prostate die with it than die from it.

Yes I do agree that screening by doing PSA is not a deciding factor but since Prostatic Specific Antigen is produced only by by prostate it is increased in only pathological conditions of prostate like Benign Enlargement of Prostate or Infection of the prostate (Prostatitis) or cancer of the prostate.

The fast rate at which it increases in cancer is pathognomic of cancer of prostate .Hence we advocate to do serial estimation of PSA every three months before taking decision for doing Ultrasound Guided Biopsy of Prostate which is the only test that can confirm Cancer of the prostate.

I do agree that it is very slow growing cancer and person die with it than from it.But it makes life miserable once it spreads to surrounding structures or to bones.

Ultrasound Guided Prostate biopsy is not a risky procedure at all.It is done under local anaesthesia as out patient procedure.

During the procedure Trans Rectal Ultrasound probe is used .Very thin and fine needle is passed through rectal wall in to prostate to get tissue of the prostate from 12-16 different sites of the prostate .

It is very accurate method to get prostate tissue for diagnosis of cancer of prostate and grade of the cancer which is very important factor to decide the line of treatment.
