How Long Does Taxol Take To Show Effect?
Question: Hello,

My mother has had two Taxols till now and today is her third. Typically, after how many weekly Taxol do you observe signs of Neuropathy in your patients?
Till now she did not get any side effects from the first two treatments. (Keeping fingers crossed).
Also, I am attaching the blood reports. Can you please opine
1. CBC, LFT & KFT before the 1st Taxol treatment (After 3 weeks of last AC)
2. CBC, LFT & KFT before her 3rd Taxol treatment
You will notice that her Lymphocytes in the first CBC report (which was taken after 3 weeks of last AC) are in the normal range, which was quite miraculous as the Lymphocytes after the AC cycles were always very low. Just after her last AC, she started taking Astragalus, Medicinal mushroom extracts and Ashwagandha, all of which are immunomodulating.
After 4 cycles of AC, her tumor has regressed to 2cm (8cm initial), and the total volume has gone down by 95%. Currently, she is saying that its close to 1cm. We are hoping for a complete pathogenic response.
Do we have tests to confirm complete pathogenic response?

Brief Answer:
she's doing well
Detailed Answer:
Hi
Thanks for follow up.
Her lymphocytes have improved. Taxol is less toxic to lymphocytes.
Neuropathy onset is variable but not usually before 6 doses.
Pathological complete response can only be confirmed on the final histopathology. Before that USG, mammogram or MRI may suggest it but never confirm.

Hi,
Were you able to check her LFT & KFT reports as well?
Also, can you please tell me what advances have been made in treating hormone positive breast cancer in last 5 to 10 years?
All the survival statistics would be at least 5 years old, so wanted to know if there have been any advancements which could result in better odds.

Brief Answer:
yes i checked
Detailed Answer:
Those reports are fine.
No recent advancement has been made in adjuvant hormonal therapy for postmenopausal patients. Standard remains aromatase inhibitors either alone for 5 years or sequentially after 2-3 years of tamoxifen.
