How Often Is Physical Exams Necessary For An Elderly Man?

Posted on Tue, 21 Jun 2016
Question: I know practices now vary re necessity of certain checks during male physicals, including frequency. Am 67; annually checked; last two or three visits have each varied (same wonderful doc). First, no frontal/hernia/genital stuff; most recently no prostate exam, either. Didn't even have my shirt off. With age, are some things just not worried about any more?

Brief Answer:
Physical exams

Detailed Answer:
Thank you for contacting HCM with your health care concerns

You recnetly had a physical by your medical provider and they did not even take off your shirt. That was not a routine thing to miss. Removing clothing is improtant to perform proper lung and abdominal exams. But also to check your skin for skin cancers. So they should have removed your shirt and did a proper skin evaluation.

Now the criteria and recommendations for prostate exam have changed recently. The digital rectal exam is not required any longer. Even PSA blood work is not recommended. The Center for disease prevention found that doing rectal exams have limited value due to missed findings and poor screening overall. The PSA is not recommended because of too many high PSA inducing doctors to perform more biopsies without finding disease but causing more harm than good. So the risk of damage from a high PSA was greater than ther actual prevention or early detection of prostate cancer.

What is more important is having a colonoscopy for screening after age 50.

As we age more disease become apparent and more diligence is needed along with better communication between patient and doctor to help maximize your health. I would discuss this with your medical provider during your next exam and check up.

I hope I answered your question. Please contact us again if you have any additional questions

Thanks for the guidance re latest thinking on prostate exam in general. I'll stop worrying about that. In the absence of symptoms/concerns I might have at my annual physical, should I just assume that hernia check and testicular exams are also things of the past for guys my age? Thanks again.

Brief Answer:
Hernia and testicular exam limited value

Detailed Answer:
Yes the hernia and testicular exams are less done mosty because by our age a hernia will be discovered on prior exam and testicular cancers only occur in young men.

Best wishes
