There is a possibility that apart from a 6 mm calculus in middle calyx of right kidney another stone might have dropped down from the kidney into the ureter which is the tube which transports urine from kidney to bladder.
If there is a stone in ureter, it could cause fairly severe pain, though an impacted stone could be silent.
So the best way to know exactly what is happening would be to get a CT scan of your KUB area - i.e. Kidneys and Bladder area. This involves radiation, so please check with the radiologist if you are suitable to undergo the procedure or not, and depending on what is noted in the scan, your treatment would be decided.
For example, if there is a stone in the ureter less than 8 mm, you can give a trial of medicines, and with improved water intake your stone will have good chances of coming out.
On the other hand, if there is stone greater than 10 mm, the chances of the stone spontaneously coming out are relatively less. If you have fever, even a small stone might need to be removed to prevent damage to the kidney due to obstruction and infection. But what we generally find is most patients do very well with conservative management, so please do not fret unnecessarily.
