Posted on Tue, 11 Nov 2014
Question: Hi i am from new XXXXXXX and on 12th april 2014 i had protected sex with a CSW at delhis red light area gb road.besides sex there was some unprotected rubbing over her undergarments.after 2 days onward i had muscle aches and pain in my legs, which gradually increased to my arms and my feet.after complete 1 week i had sore throat and cold for 3-4 days.then i had diziness, weakness, low energy which is also there till date.in may i also had cuts on my lips and ulcers XXXXXXX onwards my bp also increased and if i worked more or drive my car for long my heart would be beat faster like i have strain on my heart.july onwards i started gettin nauseatic feeling and my apetite also decreased.all this while i was having very low energy and fatigue.in august i tested positive for chlamydia in my tests.for which i have taken medicine for a week and now it is not there.i hav also suffered neauropathic symptoms like numbness in hands and feet and even sometimes burning sensation in my feet.till date i have done 20 antibody tests for hiv and 4 hiv 1 pcr tests, but all have come back as negative.my vitamin b 12 and vitamin d are also coming low.but i did cd4 and cd8 tests..reports in july came cd4 1380 and cd8 1950 and cd4 %at 30.71%..so cd4/cd8 ratio was 0.7(below 1 which is normally in hiv positive people) and today i also did cd4 cd8 test again and ratio decreased to 0.64 with cd4 at 1170 and cd8 at 1810 and cd4 percentage at 30.15%pls doctor suggest me what is the exact problem and if it is hiv more then 6 months have passed why it is not coming on tests.pls suggest me what to do as my energy levels have not been the same and why cd8 are so high(that majorly happens in hiv).what tests and what should i be doing.

Brief Answer:
HIV non reactive, Take multivitamins....

Detailed Answer:
Hello XXXX ,
I understand your concern. I am Dr. Arun Tank, infectious disease specialist, answering your concern.

- As you had performed protected sex, there is no possibility of HIV in you. This fact is strengthened by the multiple HIV tests (including gold standard PCR test). So there is no point of worry for HIV. You surely HIV non reactive person.

- All your other sign and symptoms are related to the anxiety and various vitamin deficiency.

- CD 4 and CD 8 counts are normal but ratio is decreased. This ratio is measure of your immunity. It is likely as you are deficient in Vitamin B12 and D, you can also deficient in other vitamins and trace elements. This deficient trace elements may cause variation in the ratio of your CD4/8 ratio.

- I suggest you to take multi-vitamin, multi-minerals and antioxidant syrups under your doctors guidance.

- Nothing much is required in your case

Glad to answer your further concern.
We wish you a best health at healthcare magic.
Thank you,

Thank you sir for answering my question..but normal range for cd8 is around 1000..so dont you think the increased value of 1800 counts for some major infection??
Sir i am also worried for htlv and hpv or some fungal infection...should i go for its testing also?

Thirdly 196 days have passed by...should i do further hiv testing..bcos the quality of condom i am bit worried.or 6 months is conclusive?

Lastly i would lile to dicuss in may end i took folic acid capsule..and u wont believe my heart beat was 140 and bp also shot up..and it took 7-8 hours for me to be normal...also i had a chocolate episode..wher i ate one cadbury crackle in XXXXXXX ..n 3 hours my heart was racing at full speed with
All my hands soaked with sweat..what all could this be...this is another reason i am worried about taking multivitamins.

Brief Answer:
HIV negative and no other testing is necessary.

Detailed Answer:
Hello XXXX ,

- CD8 count is not much helpful in HIV. It is the ratio of the CD4/8 which is important. If your total Lymphocyte count is increased than it is suspected of infection. Some numerical variation is posible in CD4 and 8 count so repeat testing after a week or fortnight will help.

- There in no need to test for HIV, fungal infections, HPV and HTLV etc. All your symptoms is related to anxiety and vitamin deficiency.

- Vitamin capsules usually do not cause scenario as you mentioned but your can take it under your doctors guidance.

- Forget about the sex incidence. Allay fear of HIV. You are HIV negative and no further testing is required. Do yoga and meditation that will help your mind relax and tension free.

Glad to answer your further concern.

No HIV,Probably some other infection requires exam

Detailed Answer:
Hello XXXX,

- As I mentioned if the total lymphocyte count is raised it is infection. So it is possible that it is infection. But it is unlikely infection of HIV.

- To detect which infection is there you need to get thoroughly examined. I advice you visit nearby doctor to get examined and get diagnosed.
