Is IgG4 Value Of 2.16 A Cause For Concern?
However, I also have:
(a) persistently enlarged and uncomfortable tonsils (not constantly, but most days, and to varying degrees); and
(b) an IGG4 level of 2.16. For some reason the ENT ordered various tests and one of them was an IGG4 test. Unfortunately he doesn't know why the test was ordered, why the number is elevated, or how we should proceed to clarify what the elevated IGG4 result means.
If someone on this forum has experience with any of these issues, would you mind offering some advice?
Many thanks.

No active treatment required if asymptomatic.
Detailed Answer:
Hi,
Thank you for your query.
1. Elevated IgG4 levels indicate a rare lymphoproliferative autoimmune disorder of unknown cause. The levels may also be raised in an acute illness.
2. Since this is an incidental finding, first routine treatment for the chronic tonsillitis should be done. No active treatment for the raised IgG4 levels is necessary at present. Repeat the test after a month.
3. Persistently elevated levels will respond to steroid treatment. A dose of prednisolone 1mg/kg/day tapered over a few weeks will suffice. A hematologist's opinion should be taken.
I hope that I have answered your query. If you have any further questions, I will be available to answer them.
Regards.
