Brief Answer:
Decision depends upon facilities available at hospital.

Detailed Answer:
It will depend upon level of injury, clinical status of patient, specialist and facilities available in hospital for optimal treatment of such patients.

If specialist or adequate facilities not available in hospital usually after primary aid patients need to transfer to higher center.

Hope this helps.

Brief Answer:
Physical examination is the most important factor.

Detailed Answer:
Sorry for delayed response.

As already mentioned it usually depends upon
- Clinical condition of patient like level of consciousness, neurological evaluation (Glasgo Coma Scale), heart rate, pulse, blood pressure, presence of active bleeding, other injuries.
Treating clinician is the appropriate person to judge all these things.
- Availability of facilities like, operation theater, anesthetist, specialist surgeon depending upon patients requirement.

Not all injuries requires urgent surgical intervention.
Also if surgery is indicated stabilization of patient depending upon clinical status of patient may require.

So it is not possible to give direct answer to your question without clinical examination of patient.

Hope this helps.
