Question: A woman, age 35, has had for 2 weeks a WBC count rise from 16,000 to 20,000. She has no pain, swelling, or unexplained bleeding. She is very fatigued, but we had figured that was from the fibromyalgia she has had for 5 yr. I am very concerned,

A raised white blood cell (WBC) count from 16,000 to 20,000 is commonly seen in infections.
The type of infection can be determined on the basis of differential blood count or count done to determine the type of cell raised.
An elevated neutrophil count is seen in bacterial infection.
Lymphocytes are raised in chronic infections and inflammation.
Fibromyalgia is a chronic inflammatory condition and is unlikely to cause an elevated white blood cell count.
It would be more informative if you could post the report of blood work up.

I sent the blood work from the Dr. office. I do not have access to the one from ER. I know it was 20,000 because that's what she was told. Thank you Dr. Puri.

Hello and welcome again,

I have gone through your blood reports.
The white blood cell count (WBC) is elevated and there is predominance of neutrophils.
This picture of suggestive of bacterial infection.
Fibromyalgia is a chronic condition and it is not responsible for high neutrophil.
However, an episode of bacterial infection can cause an elevation of neutrophils.
Common conditions which cause elevated neutrophils are appendicitis, salpingitis, peumonitis, boils, furunculosis, etc.
Other non infectious conditions are acute blood loss, stress, steroid therapy.
I suggest you to consult your primary healthcare provider to look for any of these conditions.
If infection is present, an course of antibiotic will resolve the problem.
