However, I very recently sustained a severely painful low back injury, unrelieved by Tylenol or NSAIDS, and I'm curious if the brief and occasional use of Vicodin would be harmful.

so, it's a potential problem (the manometry pictures in the article are going to look REAL familiar).
the other point is that treating pain... is not as good as actually getting rid of the problem. Medications working on the dysmotility would be better
metoclopramide
possibly baclofen (muscle relaxant).
That being said, these are highly variable diseases/conditions/drug effects and it would be reasonable to try .. pretty much anything. None of the side effects are either fatal nor irreversible; you might have worse dysmotility, but you might not witht he vicoden and just have pain relief.
