Is Radical Hysterectomy Necessary For Patient With Grade-II Endometriod Adenocarcinoma?

Question: Hi Doctor....My mother 65/F has been diagnosed with Endometroid Adenocarcinoma Grade-II through the endometrial biopsy. She was being treated for post menopausal bleeding and underwent hysterescopy and D &C procedure. After seeing the endometrial biopsy report ,the Gynae deptt. has referred us to Oncology Deptt. and the Oncologist after seeing the biopsy report and a further MRI test has recommended a Radical hHsterectomy to be followed by some therapy. My mother is also diabetic since 5 years and has been on glempreide and metformin. She has also had Laparoscopic Cholesystectomy two and a half years back. Now after knowing about radical hysterectomy and then therapy ...though chemo or radiation I dont know ....I am very worried since it is a major surgery. I want to know How safe is this procedure in the age of 65 and is radical hysterectomy necessary in my mother's case. I am uploading the Biopsy and MRI rest report....Thnks ...Pl reply urgently.....

Hello XXXX,
Your mother"s medical history shows, she is suffering from endometriod adenocarcinoma grade-ii. Amongst different histopathological varieties of endometrial cancers, this variety has good prognosis.
Radical hysterectomy is vague term and correct term for this tumor is Type I hysterectomy with pelvic and para-aortic lymphadenectomy. Any trained Gynec-oncologist can do this surgery.
After surgery if it will be stage I tumor she may not need any further treatment [no need of radiation or chemo].
Your mother is probably fit [which will be determined by treating team] for this surgery.
Please note that if at all any cure is there for this tumor it is surgery only,no radiotherapy or chemotherapy can cure this tumor.

Thanks Doctor...I have been told that there will be a cut in the abdomen for the surgery since laparoscopic procedure is not possible ....So I m worried how much time my mom will take to recover completely considering she is 65 and also diabetic since 5 years

Hello,
It is routine surgery for an experienced gyn-oncologist.If there are no post-operative complications recovery should be smooth.
My patients usually recover within 2 weeks and complete recovery in a months time,although standard teaching used to be 3 months for complete recovery.This is because irrespective of whether open or laparoscopic surgery the complete healing process [scar becoming strong by collagen fibres] takes about 3 months.
Laparoscopy is better in terms of post-operative pain,cosmetic scar etc.only.
Age 65 and diabetes are routinely managed in modern medicine.
