Thanks for the query to hcm,
The fact that one ovary has a cyst which is more than 4cm in size and has intracavitary papillary soft tissues or septations which may indicate a ovarian carcinoma so it has to be ruled out first. You should get ovarian tumour markers done before ( CA 125, CEA, AFP, B-HCG).
The other ovary is dermoid and both tubes are swollen with hydrosalpinx and the presence of ascites indicates a probable spread of ovarian neoplasia. So as there is no fertility concerns at your end, you should undergo TAH+ BSO through laparotomy + frozen section during surgery and if atypical or malignant cells found or malignancy detected then staging should be done at the same time with bilateral pelvic lymphadenectomy and bilateral para-aortic lymph node dissection with complete omentectomy.
This is the complete operation which you need to discuss with your gynaecologist and take an informed decision.
