Is Severe Pain In The Hip Associated With Perthes' Disease?

Question: My 16 year old daughter had perthes disease when she was 2. She had it in both side but the right was the worst. The femoral head had completely flattened out. Her doctor at the time did a let's keep an eye on it treatment. Well here we are 14 years later and she is having severe pain in the right side. Could it be that the perthes has returned? We live in rural Wyoming so there are not a lot of doctors knowledgeable on the disease. Back then the ER doc that found it said she need a hip replacement so I'm really kinda freaking out now. Any help will be appreciated.

It is highly unlikely for recurrent Perthes disease in a 16-year-old. The most common causes of hip pain in adolescents are transient synovitis (caused by viral infection) or slipped capito femoral epiphysis (SCFE). SCFE occurs due to slipping of the head of femur bone.

Transient synovitis is treated with NSAIDs like Ibuprofen and eventually subside on its own. SCFE will require surgery. Here the doctor will fix the slipped bone with the help of a screw through a small incision.

X-ray is required as the next step. Depending on the Xray result, MRI is indicated.
