Evaluation of discharge and increase of insulin dose
Detailed Answer:
Hello Mr XXXX, Her postprandial (2-hour) sugars are still high as the target after 2 hours is 120 mg/DL.
So in my opinion she needs dose titration.
At the same time this discharge needs physical evaluation.
Usually vaginal discharge starts increasing at this gestation and is physiological up to around 2-3 teaspoons a day ( very rough and crude description to make you understand though I know it cannot be measured).
Moreover the discharge at this stage is little mucoid and thick in consistency.
But any increased discharge at any gestation needs evaluation.
Do whole day sugar levels as fasting, 2-hour post breakfast, 2- hour post lunch and 2- hour post dinner and then visit your doctor for dose titration (keep taking the normal dose of insulin you have been taking so far).
All the best.
Thank you so much for your reply. she met endocrinology doctor and OBgyn doctor yesterday. As per the endocrinology doctor postprandial after one hour the normal is 140 mg/ DL. As per him she is always checking 1 hour after food. regarding the discharge case i will forward your reply to my wife. thanks a lot for your advice

Normal if done after 1 hour
Detailed Answer:
Yes dear your endocrinologist is right as 140 mg/DL is normal if you are doing sugar after 1 hour of meals.
But general recommendations ask for 2-hour postprandial sugars (what I practice)
but both are good if the values are normal.
All the best.
In case you have any other question please feel free to discuss.

Most welcome dear
Detailed Answer:
Most welcome dear. All the best.
In general the fundamental feature of abnormal discharge is excess of itching and foul smell.
If none is there and quantity is minimal as told then nothing to worry.
