Neck And Back Pain, Burning Soles Of Feet, Heaviness In Legs, Done Blood Test, X-ray Showed Early Degenerative Changes In Disc. Further Treatment?

Question: Dear Doctor,

I am 53 years old working as a Secretary in a Fertilizer factory. I used to work more time on computers as part of my job.
I have been suffering from neck pain along with mild upper back & lower back pain for the
last 6 months. At that time Doctor Diagnosis me for cervical spondylitis after taking X-ray and given medicine for 15 days and also advised for some exercise . After that there was control in the pain, but mild pain continued. I used to take counter NSAIDs whenever there is increase in the pain in the neck on need basis and also use ointments. This continued for some time. Again the pain increased in the neck this month and I have again taken X-ray - Cervical Spine Ap/Lat. vie on 3/6/2012. The report is reproduced below for your kind perusal .
SOFT TISSUE : prevertebral soft tissue opacity is seen normally
BONES : - no significant alteration of the vertebral curvature is seen.
vertebrae are normal in density height and outline.
Anterior & posterior arch of the atlas is normal. Odontoid peg is normal.
Small bilateral cervical ribs are seen. Mild anterior lipping is seen at the
level of the C6-7 vertebrae.
DISC SPACE: disc space is normal except at C6-7 vertebral level where it
is mildly decreased.
OPINION: SMALL BILATERAL CERVICAL RIBS ARE SEEN. EARLY OSEOARTHRITIC
CHANGES ARE SEEN IN THE C6-7 VERTEBRAE. DISC SPACE IS MILDLY DECREASED
AT THE C6-7 VERTEBRAE. DISC IS MILDLY DECREASED AT THE C6-7 VERTEBRAL.
LEVEL SUGGESTIVE OF EARLY DEGENERATIVE CHANGES IN THE DISC.

In addition to above, neck pain/ back pain I have been suffering from burning sensation in the sole of both feet for the last one year for which I am taking B-complex tablets now and then, but of no use. There is also heaviness in my both legs.

My Fasting blood glucose level is fluctuating between 100 to 131 during the last one year. Values are
100, 119, 129, 131. Report on 3/6/12 is 129 mg/dl. I am not taking any medicine for Sugar control. As per Dcotors advise I am taking Envas- 5 mg for blood pressure and BP is under control 120/80. It usally goes to 140/90 without medicine. I have also problem of indigestion and acidity.

Kindly go through my above X-ray report, back ground and other details and advise me for further treatment/medical check up/medicine to be taken.

Thanks for posting your query.
The X-ray of cervical spine is suggestive of presence of cervical ribs and early spondylottic changes at the level of C6-7. These are responsible for the pain in neck and upper back.
The pain the legs and heaviness may be related to similar degenerative changes in the lumbar spine. An MRI of the spine will help in the exact diagnosis.
Such degenerative changes are rapidly progressive in the presence of an uncontrolled diabetes.
You need to consult a physician for proper control of your blood glucose levels with medicines.
For the symptoms you are having, you need a combination of pain killer and muscle relaxant after examination by your orthopedician. Taking calcium supplements and improvising your posture will also be helpful.
Regular physiotherapy and exercises are recommended to decrease the progression of your problem.

Dear Doctor sir,
Thanks for the prompt reply and advise.
I have already taken advice of Physician for sugar control. Doctor has not advised at present, medicine for sugar control as it is in the range of 100 to 130 mg/dl and value of 70 - 130 is considered normal. He advised to start medicine if it cross 130 in further 2/3 tests. He recommended walking & exercise and to avoid sugar intake only for the present.

I forget to mention that my Rheumatoid factor test report is REACTIVE. In the mean time, I request you to suggest some muscle relaxants & pain killers locally available to control pain which will not upset my stomach as I have indigestion, acidity and some times heartburn problems also.
Kindly reply & prescribe medicine.

Hello.
Thanks for writing again.
The pain killers and muscle relaxants are prescription drugs and since you have a problem of gastric upset, these need a regular monitoring and dosage regulation which is not possible online without the benefit of actual examination.
Since I haven’t physically examined you, it is inappropriate and unethical to
prescribe you scheduled drugs that you can get locally. I am bound by the rules of law to not to give any online prescriptions. Hence you will have to see the Orthopedician for the medicines that you require.
