Question: This all started in November of last year. One day I was sitting at my desk when out of nowhere I experienced a lightheaded dizziness and chest pains that felt as if someone where squeezing my upper body. I went to the ER and they gave my I've fluids did blood work, said I was fine and sent me home. One week later I still wasn't feeling well so I went to my regular doctor who sent me to a cardiologist due to a possible irregular EKG. I went and after a stress test and echo they said my heart was fine. Three days later I began to experience tremendous pressure on the right side of my head, pressure in my ears and numbness on the right side of my face, I went back to the ER and after 7 hours they told me it was all stress related. Which I know is not the case. Within a day or two I began to experience numbness/pins and needles in my right hand mainly the pinky, ring, and middle fingers, as well as my right foot and still had the head pressure on the right side. The neurologist sent me for a brain MRI and a C-spine MRI. They did find Chiari 1 malformation and a bulged disk in my neck. I followed up with a neuro surgeon who said I did not need surgery, a second surgeon wanted to operate on everything even though my neurologist and 1st neurosurgeon said I didn't need any surgery. I don't feel my symptoms are due to the Chiari malformation or my slipped disk. I am not in any pain in my neck and from what I've read about Chiari malformation it shouldnt have just hit me out of nowhere. Both the neck and Chiari would have been things that would have progressively gotten worse over time.

I've seen an ENT to rule out sinus problems, I had sinus surgery and septoplasty in 2007. My sinuses are fine, ears are clear. I am seeing a new GP who just did a bunch of blood work, all came back normal, thyroid is normal, had an ultrasound of my thyroid it's fine. Within the past month my blood pressure has started to get borderline high 130/80, 140/80, my cholesterol is 202 I'd never had it this high before not that it's that high but it's a change. My doctor wants to put me on migraine medication and a beta blocker Bp med to see if it helps. I haven't started the meds yet because in my heart I don't think this is the problem because migraines wouldn't be causing all these symptoms. All my symptoms are:

Pressure headache on right side only, comes and goes but is there quite often
Numbness in fingers of right hand (had an EMG, no significant carpal tunnel)
Numbness in right foot, comes and goes
Chest pains, comes and goes
Pain in right thigh
Dizziness
Swollen tongue
Wrist pain, mainly right wrist, feels almost like veins are swollen
Pain on sides of neck, mainly right side, near where carotid artery is
Temple pain, right side
If I touch a spot on my head above right temple I will feel a shooting pain to the center of my head. Constant!
Veins on top of right hand seem very blue.
Shortness of breath just climbing stairs
When this all started I had an extreme loss of appetite, lost 10 lbs

I am 31 years old, 5'8", 143lbs, married mother of 2 boys 12 years and 18 months. Quit smoking 6 years ago. Work as a secretary. Please someone help me, tell me where to go next. Could this be a vascular or circulatory problem? I'm terrified that no one has been able to figure out what's wrong. I now my body, it's not stress, there's something wrong. Please help. Thank you.

Hi, welcome and thanks for writing in.
I have gone through your history. It doesn't look like a vascular or circulatory problem. In vascular problem there should be generalised involvement of vessels which will pick up in CT AND MRI SCAN.
One more important point is that it is not due to chiary malformations or disc, so need to get operated.
Though you didn't feel it is stress related, it looks like impending doom. In such condition tightness of chest and heaviness of head and all other symptoms comes and go. It is early signs of possible panic attacks.
You have nearly completed all your possible investigations. So don't do more now. I will suggest please consult board certified counsellor for your symptoms. It is best approach in this scenario because every time you visit different doctor he will advise few investigations and new medicine which is not working at all.

Hope I have answered your query. If you have further concern, please let me know.

Regards.
