Should Probotic Supplements Be Taken Before Or After Food?
Question: Dear Doctor,
When does one take 'Lacto bacillus sporogenes and other probiotic' capsules: before, after or between meals? I get hangover like headaches every other day. I feel sugar containing foods trigger the same. My Doctor is not able to find the real cause. Pl see if u can suggest some remedy.

Brief Answer:
Have an upper gastrointestinal endoscopy
Detailed Answer:
Hi,Sir,
I have read your query and understand your concern.
Upper abdominal bloating and belching point more towards Gastroesophageal reflux disease.
Probiotics may be taken with or without food but they are of hardly any help in this regard.
Sugar foods can lower the esophageal sphinteric pressure and can exacerbate the acid reflux.
I advice you to have an upper gastrointestinal endoscopy with H.pylori testing.
Meanwhile i recommend tab esmoprazole 20mg once a day for 14days.
This drug should be taken under strict medical supervision.
I hope this info is helpful to you,
Thank you

Thnx Dr. My problem is "hangover like headaches starting at around 4 am especially after eating sugar containing foods at dinner. the Hangover like Headache and giddiness remains for whole day, accompanied by Gas formation in upper gut. Stool test is ACIDIC whereas normally it should be alkaline. Blood tests sugar etc are normal. I don't suffer any oesophageal or acid reflux problem as mentioned by you. CBC is 1,65,000. Can my gas problem be due to low acid production in stomach with age, resulting in gas formation due to fermentation and consequent hangover like headache. Or is it fermentation of sugars due to candida infection in the duodenum. I am slim 64 kg 5' 7'' tall.
Brief Answer:
Bacterial overgrowth syndrome could be a possibility.
Detailed Answer:
Hi Sir
I have carefully read your query and understand your concern.
In case if you are very sure if it’s not GERD, Then a consideration has to be given for Bacterial overgrowth syndrome/BOS in view of acidic stool ph.
D lactic acidosis syndrome can be a part of malabsorption particularly after intake of sugary foods over night.
I recommend you to have a urine/blood D lactic acid levels measured.
A colonoscopy can help to completely examine the lower gut for the source of malabsorption and other organic lesions if any.
A 7-10 day course with antibiotics like Ciprofloxacin and Metronidazole can be every effective in treating this condition.
Note these drugs have to be taken under strict medical prescription.
Hangovers headache can be part of cluster/mixed migraines headaches.
Drugs like Triptans have an excellent activity against this form of headache.
Consult your doctor with reference to above drugs.
Thank you
