Regarding your concern, I would explain that water retention in her clinical situation could be related to heart failure.
Unfortunately there are no OTC to reduce water retention.
The only thing to do is to reduce salt and water intake.
If she needs something for the pain, acetaminophen (tylenol) would be safe if taken concomitantly with warfarin.
Coming to this point, I would recommend consulting with her attending physician for a careful physical exam and some tests:
- a resting ECG and cardiac ultrasound to examine her cardiac function and structure
- NT-proBNP
- coagulation tests (INR)
- other blood lab tests (complete blood count, PCR, sedimentation rate, blood electrolytes, fasting glucose, kidney and liver function tests).
You should discuss with her doctor on the possibility of optimizing her therapy. Diuretics may be needed in this clinical situation, but they are given only under medical prescription.
