Question: Dear doctor - was surfing net to check medicines for EPS and came across pramipexole. I have following questions:
Can I replace kemadrin with it?
Does it help with eps? If yes, at what dose?
Does it have any stomach related issues that I face with kemadrin?

Pramipaxole is a dopaminergic drug and the drug is commonly used for Parkinson's disease. I am not sure about the use of this drug in EPS. Yes theoretically this can help in EPS and is even used by some doctors for drug induced EPS.

I don't think replacing this for Kemadrin is a good idea. Stopping Kemadrin will not cause much EPS considering your other drugs.

Yes this can help in EPS but low doses should be used. 0.5 to 1 mg can be used.

Pramipaxole doesn't cause much stomach issues. The drug is used even by a lot of psychiatrists for resistant depression symptoms.

Depending upon the replies i got from you, met my doctor today and discussed in detail. According to him:
Isopropramide does not help with EPS, is it true? I am confused and worried about EPS because stelabid 3 mg in the morning is helping me with anxiety and if it causes EPS to me, i will have to stop this (as kemadrin and pacitane don't suit me) and it will bring my anxiety back.
According to him, pramipaxole can help with EPS to an extent but not same as pacitane or kemadrin but he is of the opinion that it will help me with the mood and anxiety also. I have following questions to ask you:
Do i need to worry about EPS if am taking stelabid without kemadrin or you think stelabid causes lesser Eps than plain trifluoperazine?
With 3 mg stelabid in the morning and 1 mg in the night, do you think pramipaxole should be help me with EPS and is 4 mg a high dose? does pramipaxole really help with eps?
Does pramipaxole cause sleep problems if taken in the night? My doc has suggested 0.125 mg of it, he said try it in the morning and if its too sedating in the morning, then take it in the night.I have read on the net that it can cause insomnia as a side effect, is it true? if yes, will taking in the morning will make me sedated? the net also says it can cause nausea, is it true? is 0.125 too high a dose to cause nausea and sleep problems?

Taking Stelabid alone in 4 mg will not cause EPS. Stelabid is better and safer compared to Trifluperazine alone.

Isopropamide is long acting anticholinergic and to some extent this helps in controlling EPS.

Yes Pramipaxole can help in EPS and as I have said the drug will help in depressive symptoms too.

So you should not get worried for EPS at 3-4 mg Stelabid.

Pramipaxole can be added. At least this will help in depressive symptoms. I am not very sure about its use in drug induced EPS. But yes the drug is very nice drug for parkinsonian symptoms. EPS also have same pathophysiology as Parkinson's symptoms.

Pramipaxole can cause drowsiness and also insomnia. This depends upon individuals to individuals. The drug is known to cause nausea and this could be troublesome. This is the reason the drug is started with low dose.

Upto 3-4 mg dose Stelabid will not cause EPS. Yes higher doses like 5 mg or more could cause mild EPS.

Young means children and young adolescents. If we consider age then the risk in you is relatively lower compared to young adolescents.

EPS is not permanents with Trifluperazine, but yes long term use could cause tar dive dyskinesia. This is difficult to treat.

Yes if you are tolerating the drug then EPS will not occur. But if the drug is used in high doses for very long time like 6 months to years then this could increase the risk of Tardive dyskinesia.

So stop worrying, you are on 3-4 mg dose and this will not cause EPS.

I was taking kemadrin 2.5 mg in the morning and 1.25 in the afternoon till yesterday and was getting indigestion and other stomach issues, didn't take it today, i have following questions to ask:
can kemadrin cause these issues? i didn't take it today and expected that stomach issues will go away but still getting stomach issues, is it too early to expect the stomach issues to go or they will go in sometime?
Can kemadrin cause stomach issues even after stopping and how long will it be before they go away?
Read on the net that kemadrin stays in your system for 60 hrs, is it true, if yes does that mean that it will take 60 hrs for my stomach issues to be alright?

Can kemadrin also have after effects, if yes how long they last?

Kemadrin can cause stomach issues. If you have stopped taking it then in about 24 hours the symptoms will start decreasing. Though the drug stays in system for long but the symptoms will decrease in a day.

Stelabid should not cause stomach issue but yes instead help because the drug reduces anxiety and also reduces acid secretion.

Good evening doctor - went to my doctor yesterday, he wants to add fluvoxamine sr 50. All other ssris give me major stomach problems, according to my doctor fluvoxamine causes the least stomach issues, is it true? Read on the net that it causes more stomach problems as compared to other ssris, am confused, need your advise. is sr formation known to cause less side effects? he has told me to take in the night, will it cause sleep problem if taken in the night? my doctor says it does not cause sleep problem, he said it helps with the sleep, is it true? apart from ocd, does it also help with anticipatory anxiety, meeting phobia and panic attacks?

Fluvoxamine SR 50 will be tolerated with ease.

Fluvoxamine 50 mg is not a high dose and should be tolerated easily. But if we consider gastrointestinal side effects and overall tolerability then based upon a number of research studies, Fluvoxamine is associated with gastric side effects in a lot of individuals. The tolerability is also less compared to other SSRIs. Citalopram or Escitalopram (which is available easily) are better tolerated.

But yes 50 mg SR formation will be tolerated easily because of sustained release of small amounts of drug in system. The drug is not associated with much sleep issues so you can take this without worries as far as sleep issue is considered.

The drug in adequate doses has great efficacy in OCD and also in anxiety or anticipatory anxiety. So you can give this drug a try.

Stelabid can be taken with or after food. There are not much effects on bioavailability because of food.

Yes you can take Lesuride with Stelabid.

Adding low dose of Lesuride will not increase the risk of EPS. Usually in dose of 150-200 mg the drug can cause EPS. But in your case I guess they have given low dose like 25-50 mg.

Low dose of Lesuride in 25-50 mg helps a lot in anxiety and resistant depression.

Yes Lesuride will help in stomach issues and this drug will also help in anxiety control because your anxiety is mainly related to anticipation and gastric issues.

Half life is around 8-9 hours. Twice a day dosing is advised.

Trifluperazine half life is around 3.6 hours but elimination half life is more.

Lesuride doesn't affect much as far as sleep in concerned. In some individuals the drug could cause mild insomnia.

have started lesuride 25 in the morning for the last two days. I have following questions:
what should be the dosage of lesuride for anxiety?
i have made amitryn 75 from 100 mg for the last two days? will it bring back my anxiety? what is the therapeutic dosage of amitryn?
i am getting a lot of acidity, hiccups and acid reflux, what can be the cause out of the medicines i am taking? is it because of amitryn, if yes, can i replace it with tynept? will tynept be able to control my depression?

Low dose of Lesuride like 25-50 mg helps in anxiety.

No reducing the dose of Amitriptyline to 75 from 100 will not result in relapse of anxiety. You are already on a number of drugs and reducing the dose would not cause any relapse.

High dose of Amitriptyline could cause gastric symptoms. But replacing this with Tynept is not a good idea. Tynept is not a strong antidepressant as Amitriptyline.

Increasing the dose of Lesuride to higher doses will only increase the risk of side effects. Better to continue to take 25-50 mg doses only.

I would have advised you to either stop Sizopin and increase the dose of Lesuride. Or I would have advised you to stop Lesuride and increase the dose of Sizopin to 25 or if needed 50 mg. Rest of medicines I would have advised same.

I usually advise my patients to take 25 mg Lesuride twice a day.

Yes continue the Stelabid as you were taking. No combining two pills will not increase the chances of EPS. Lesuride in high doses might result in EPS. But low doses are safe.

Both are relatively safer but Stelabid in high dose is more prone to cause EPS. Stelabid is more commonly used in anxiety. Yes Lesuride will help in anticipatory anxiety too because this helps in reducing the anxiety.

Amitriptyline is used in 25 to 50 mg doses usually. Some times in depression higher doses are needed.

I went to a gasteroentrologist for my stomach problem, he has asked to take nupenta cp ( pantaprazole 40 mg and cinitapride 3mg slow release) in the morning and ganatol total ( itopride 150mg and pantaprazole 40 mg slow release) in the night, i have been taking this for the last 3-4 days, there is slight respite but not much. He said to add rebonik plus ( rabeprazole 20mg and lesuride 75 slow release) if ganaton total does not help, i have following questions:
since rabonik plus contains lesuride 75 sr, can i start taking it along with stelabid, would it cause any eps or sleep problem?

You are on a number of medicines and these medicines are known to result in heart burn, hyperacidity etc symptoms.

Lesuride will help in reducing the gastric symptoms effectively. Lesuride is commonly used in gastric conditions.

Nupenta CP has cintapride and this is a gastric motility enhancing agent. The drug will definitely help you in improving the gastric symptoms.

You can even start taking Lesuride 75 mg and this will not cause any EPS or much sleep issues. But don't increase the dose to more than 100 mg a day because this might affect its depression and anti anxiety properties.

Rabonic plus should be better taken in morning preferably before breakfast.

Stelabid might cause constipation but you are not taking very high doses.

You can continue to take Stelabid as advised and adding Lesuride 75 would not increase any risk of EPS. No need to stop Stelabid.

I took lesuride in the night day before and was feeling slightly drowsy and fatigued and today i took in the morning and it made me very drowsy, sleepy and fatigued. I am thinking of continuing it in the night only. How long these side effects like drowsiness, sleepy, fatigue and responses being slow will last? No I don't think this is due to Amitriptyline. You have reduced the dose from 85 to 75 and this is a very low dose. This will not affect in any manner.

You can continue to take Amitriptyline at 75 mg dose without any fear.

Amitriptyline is good considering your symptoms. Tynept OD in past didn't work for you. If even after this you want to start Tynept then cross tapering is best option.

You were taking Amitriptyline for long and I don't think this resulted in stomach issues now. Still if your doctor thinks then that is right. You can reduce the dose of Amitriptyline without any worries.

50 mg is not a low dose but yes, since the dose is less Antidepressant effect will decrease by reducing the dose.

You can try Tianeptine 12.5 mg in morning and this should compensate for reduced doses of Amitriptyline.

Yes you can take Tianeptine and Amitriptyline together. No need to worry.

dear doctor - with lesuride 75 od in the night, my anxiety is better but the thoughts of how will i speak in the meeting, will i fumble etc. keep on bothering me. Yes the slight exacerbation in anxiety happened probably because you didn't take Betacap TR and Stelabid. If you don't want to take excess Stelabid then you can take Betacap TR alone to keep your anxiety under control.

Starting Fluvoxamine could worsen the gastric symptoms and anxiety. Yes the drug is implicated in causing more gastric symptoms compared to other SSRIs.

The dose of Lesuride can be increased to 100 mg OD but dose shouldn't be increased more than that because higher doses would have more antipsychotic effects rather than positive effects on mood and depression.

You can increase the dose of Stelabid for sometime. No need to worry. Increasing the dose would help in reducing the anxiety. Yes doses higher than 3 mg would have antipsychotic effects.

Oxazepam 10 mg is good option. The drug is safer than Zapiz and is less addicting. There is risk of tolerance but since its sedation is relatively less so risk of abuse and dependence is very low.

You couldn't sleep properly most likely because you have exacerbation of anxiety right now. This could be the reasons for sleep issues. Lesuride has no direct effects on sleep, in some individuals the drug causes sedation while in others it might result in poor sleep.

Exacerbation of anxiety is the reason that you are not responding to high dose of Stelabid. For some time you could take Propranolol or Oxazepam for relief of symptoms.

Fluvoxamine helps in social anxiety and anticipatory anxiety too. The drug is advised to be taken in twice daily doses. So you can take it in morning or evening. The drug helps in sleep in most individuals.

Oxazepam could cause memory issues or cognitive impairment like any other Benzodiazepine drug. This doesn't mean that you will have memory issues. The withdrawal risk is there but is lesser that Zapiz. Its not advisable to be taken regularly for more than 4 weeks unless strongly indicated. The drug is good for stomach. Oxazepam by relieving anxiety helps in sleep, though its not a very strong hypnotic agent.

Yes Fluvoxamine is mainly used on OCD cases. But sometimes the drug is also useful in anxiety. The drug can help to control the symptoms you have but higher doses will be needed to get remission. Higher doses will increase the gastric side effects and at the end the drug will not prove useful.

In my opinion 50 mg is not enough dose and high doses should be given. Yes efficacy wise Fluvoxamine is equal to Escitalopram or Paroxetine.

The drug should show effects in around 2-4 weeks.

Good evening doctor - my medication is as under:
quetiapine 100, quetiapine sr 100, sizopin 12.5, amitryn 60, lesuride od 75, fluvoxamine cr50, alprax 0.25 in the night
betacaptr40, stelabid 1mg in the morning
since the time fluvoxamine has been added my stomach has been behaving weirdly and feel sedated in the morning, don't feel like leaving the bed and have been waking up at 12 and 1 in the afternoon
went to my doctor, he has asked me to stop fluvoxamine started pramipexole 0.375 er in the night, i have following questions:
i have been taking fluvoxamine for the last 15 days, will stopping it abruptly cause withdrawal symptoms?
will premipexole take care of my depressive thoughts and help me with anxiety?
can pramipexole cause stomach problems?
do you think its a good idea to stop fluvoxamine and add pramipexole or something else should be added?
was reading on the net that pramipexole helps with parkinson and restless leg syndrome, does it also act as antidepressant and anxiolytic agent?

No stopping Fluvoxamine will not cause withdrawal symptoms. You were taking this drug for 15 days only and even abrupt stopping will not cause withdrawal.

Pramipaxole will definitely take care of depressive symptoms. Pramipaxole is well tolerated if we consider abdominal symptoms.

Pramipaxole is a drug that helps in resistant depression and should be given a try. The drug is primarily used in Parkinson's disease but has great efficacy in resistant depression.

The dose of 0.375 mg is fine. Normally for resistant depression and even anxiety symptoms dose of 0.375 to 0.5 mg is given. This will start showing its effects in around 2-4 weeks.

The drug helps in anxiety and panic attacks because some hypothesis state the role of Dopaminergic system in causation of anxiety and Pramipaxole alters dopamine neurotransmission. But its primary effects are on depressive symptoms.

Lesuride works better for stomach symptoms while for anxiety Stelabid is good.

Increasing the dose of Lesuride to 100 or should be avoided. Upto 100 mg the drug helps to improve the mood and has great roles in depression. Don't stop Lesuride and also continue to take Stelabid.

I haven't heard about Inositol for use in anxiety but if you want I can read studies on it and will get back to you.

good evening doctor - thanks for your email. i did not take stelabid 1 mg in the morning today, instead i took 25mg lesuride, my anxiety was little better today but stomach was not that great. i am thinking of stopping stelabid totally. I have following questions to ask:
i am taking lesuride od 75 mg in the night, along with that it is ok to add 25mg in the morning and also take 25mg sos depending upon my stomach? checked with my doctor he said ok as according to him upto 200mg can be taken for depression and the net says unto 300 mg can be taken for depression and as per you its 100mg? am confused whether i can take along with 75 od, 25mg in the morning and 25mg or may be 12.5 in the afternoon and will it still work as an antidepressant? also am not not taking 100mg, or 200mg in one go, am spreading it throughout the day? will spreading a larger dose have antidepressant effect compared to taking it in one go, what i mean is, does 125mg in one go work the same way as 75mg od in night and 25/25 mor/afternoon? or taking 50mg in the morning along with 75od in the night an option? will 50 in the morning help the stomach better or 25/25 mor/afternoon?

Lesuride in low doses has more dopaminergic actions and thus helps in improving the positive symptoms. When the doses are increased progressively then the drug becomes more antipsychotic drug. At high doses it has Dopamine blocking actions and these doses helps to augment the actions of other antidepressants so this can be taken in amount and it has been said that even 200-300 mg is antidepressant dose. Best actions are seen in doses upto 100 mg as per my experience.

You can stop Stelabid and can add 25 mg Lesuride in morning and 25 mg on SOS can be taken. No need to worry. Best is to take 50 mg twice a day and if that is not possible because of drowsiness then 25 in morning and 75 mg in evening.

The risk of stomach issues with Pramipaxole is less. The other drugs you are taking are also safer if we consider abdominal side effects. You can continue to take these drugs. But you are on Lesuride, Quetiapine and Sizopine that is three antipsychotics which is not a good idea, we discussed about this before too.

Lesuride has not much effects on sleep quality and quantity and if you couldn't sleep that was due to some other reasons. If you couldn't sleep then you can increase the dose of either Quetiapine or Clozapine for some time. This will improve the sleep.

I don't think that drug change cause the sleep issues. To some extent psychological reasons are also responsible for poor sleep you had.

Increasing the dose of Pramipaxole is a good idea. Half life of Pramipaxole is around 8-10 hours.

Pramipaxole usually doesn't cause sleep problem. But continue to take Pramipaxole for some time and you will definitely see improvement in your depression.

Hello doctor - last night as advised by my doctor, i took pramipexole 0.125 normal along with 0.375 er, it helped me to sleep faster, went to sleep at 11.30pm and woke up at 2.30 am, could not sleep after that, i took one more pramipexole 0.125 at 2.30 am, still was wide awake and then took 0.25 alprax, i think after that i slept for sometime with great difficulty. My other medications are :quetiapine 100, quetiapine sr 100, sizopin 12.5, amitryn 60, lesuride 50, alprax 0.25, pramipexole 0.375 in the night

Please avoid taking repeated dose of Pramipaxole as this drug is for depression and no for sleep. If you couldn't sleep then you should have taken Alprax or Clonazepam.

You couldn't sleep probably due to anxiety. Try to ignore this single isolated incident.

I don't think Leauride is culprit. The drug has not much adverse effects on sleep.

Remember that Pramipaxole is not for sleep. The drug is a treatment for resistant depression. Normally for depression dose of 0.5 mg is good but it can be increased to even 1 mg.

For some time you can take clonazepam on SOS basis for sleep.

since i got a late reply from you, went to my doctor and he said to increase quitipin to125 from 100 and it did the trick for my sleep. Also, because of the weekend, i made amitryn 50 from 60 and my stomach is feeling better but when i woke up in the morning was feeling little depressed, i have following questions:
- since i have reduced my amitryn, can i increase premix to counter my morning depression? my anticipatory anxiety is still there, will increasing premix help me with that?
- i am taking premix er 0.375 and 0.125 normal, how much should i increase it by?
- it comes in 0.375 and 0.75 in er and 50 in er is not available? should i increase night dose by another 0.125? i mean should i take 0.375 er +0.25 in the night? or make it 0.375 er+0.5? if i increase normal dose along with er 0.375 in the night, will it have effect next full day? or make it 0.75 er in the night?
- as he half life of premix of 9-10 hrs, will increasing normal premix in the night, will help me better with my day time anxiety?

since i got your reply late, took extra premix (0.375er+0.125+0.125), slept with a lot of weird and scary dreams, was wondering if it because of increased dose of premix?
since i have reduced amitryn from 60 to 50, will it increase my anxiety because of the reduced dosage or premix will take care of it? because of the reduced dosage of amitryn, i increased dosage of premix, do you think it is required or premix 0.5 will take care of it? after reducing amitryn to 50, my stomach is better, therefore, not keen to make it 60.

Reducing the Amitriptyline to 50 from 60 will not increase your anxiety. Pramipaxole will take care of any anxiety or depressive symptoms. Yes regular use of Pramipaxole will help you to remain relaxed and effects will be sustained.

Please don't think to jump the drug dose. This will only increase the risk of side effects. Remain on 0.5 mg Pramipax for some days and let this dose do its work and this will control the depressive symptoms.

dear doctor - do lesuride or pramipexol cause breathlessness? i had to give a speech today and took inderal 10 and zapiz 0.25 before it, despite that i felt breathless and had to cut short my speech. earlier in my presentations zapiz 0.25 along with inderal 10 would help me, today even they failed. i have following questions:
- checked on the net that premix can cause shortness of breath, i am 0.375 er + 0.125 normal, is it true? or lesuride can cause it, the net does not talk about it though

-when i wake up in the morning, i get headache, can't figure out its bcoz of what, the net though says that premix can cause it? is it true?

You can take 20 mg Inderal in morning and afternoon. There should be not problem in that. Betacap TR can be stopped.

Thank you for your reply. i took lesuride 75od in the morning today, throughout the day my movements were slow, was feeling drowsy and little low also. i have following questions:

Its better if you take 50 mg twice a day, this is what I usually advise to my patients. This will not cause day time drowsiness and if you have some drowsiness that will go away in few days.

Its because of Lesuride and Pramipax. Pramipax might have supplemented the actions of Lesuride and other antidepressants.

Pramipax might make you drowsiness in morning so its better you continue to take it in evening.

Stelabid controls anxiety but for stomach symptoms better to take Lesuride.

i have following symptoms:
breathlessness while talking to people and i have had anxiety attack in front of my boss and whenever i have to meet him, i feel anxious, nervous and breathlessness, earlier i was taking lesuride and with that also breathlessness would happen, my doc says its because of anxiety, what can i do to curb it, do you think stelabid is an option and if yes at what dose?
my hands feel shaky, there are no tremors but feeling of shakiness, do you think increased dose of quitipin or sizopin or alprax causing it, i have started feeling that since the dose increase of these medicines? pacitane, kemradin don't suit me as they give me very bad stomach, earlier i was taking beta cap tr 40, i have stopped that as i did not feel it making any difference, what should i do? do you think beta cap will help, should i take beta cap la or normal and at what dose?
does premix cause tremors? do you think increased dose of premix will help me with tremors and anxiety? if yes, how much should i increase the dose by and in the morning or evening?

The breathlessness you are having while talking to your boss of another individual is due to anxiety only. Anticipatory anxiety is the reason for these symptoms.

Adding Stelabid to your prescription at this point of time is not a very good idea. You are on 300 mg Quetiapine in total, Amitriptyline 60 mg, Sizopin 25 mg and Alprazolam 0.375 mg. Adding Stelabid could increase the risk of side effects. For shaking or anxiety you can think of starting Propranolol or Betacap in 40 mg dose. If that didn't work then combination of Propranolol and Clonazepam can be tried.

Pramipax at 0.375 doesn't cause tremors. You can increase the dose to 0.5 mg if needed after talking with your doctor. This will help in better symptoms control.

i took beta cap tr 40 in the morning, my shakiness was better. along with premix er0.375, i took 0.125 also in the morning, anxiety was slightly better but was there. i have following questions to ask:
