Hi,
Thanks for posting the query on HCM. After going through your query, I would like to comment the following:
1. A 5 mm lesion in the right upper lobe needs to be evaluated. A subcentimetric lesion of such manner can be kept under observation for 2 years and if it is static then it can be labelled as benign. However there are many other factor to be considered. A whole body pet scan normally pick up any possible malignant focus in the body. Excision biopsy / open lung biopsy is also an option.
2. A confirmed diagnosis can be had only on histopathological examination.
3. The further line of management depends on the arrived diagnosis.
4. Sometimes the lesions can be a sequelea of old healed infection.
5. Thus after all the ancilliary investigations have been done there are only two approaches one is observation and the other is removal of the lesion. Your physician must have taken the decision after taking all the factors into consideration.

Yes, excision of the lesion is an acceptable option. Also after excision, histopathological examination of the lesion will help diagnose the exact etiology of the lesion.
