Suggest Dosage Of Augmentin DDS, Ascoril-LS And Combiflam

Question: Dear Dr, My 18 month old is having high temperature fever at night. He also has cough and running nose throughout the day since day before yesterday. But he is active during day time. His temperature went upto 101+ yesterday night. Dr. diagnosed him with URTI and suggested to give him Augmentin DDS 2.5ml BD, Ascoril CS 5ml TDS for 5 days. And combiflam 5ml in case of fever. Is the medicine and dosage appropriate for this condition. Is there is any side effect to these medicines? thanks.

The average weight of 18 months old baby is around 10-13 kgs.The Augmentin DDS is an antibiotic and each 5 ml contains 900 mg of Amoxycillin(800 mg) and clavulinic acid(100 mg).
The dose of amoxycillin is 50-100 mg/kg/day in two to three divided dosage depending on the severity of infection.So the dose of Augmentin DDS 2.5 ML twice a day(total 5 ml-800 mg) is appropriate for age.
I would like to know whether the prescribed syrup is Asoril C or Ascoril LS.Both syrups are used in cough but composition is different and prescribed depending on whether the cough is productive or nonproductive.The dose of 5 ml three times a day is appropriate for both the syrups.
Each 5 ml of Combiflam contains 100 mg of ibuprofen and 162.5 mg of paracetamol.The dose of paracetamol is 10-15 mg/kg/dose and ibuprofen about 10 mg/kg/dose.So 5 ml of each dose of combiflam is also appropriate for your baby.
Amoxycillin can cause loose stool and it is better to give probiotics(helps in maintaining gut flora)along with the augmentin to avoid the side effect.
Asoril C contains codeine which is cough suppressant and Asoril LS contains levosalbutamol which is a bronchodilator.The codeine may cause sedation if given in high dosage.Observe the baby and if she gets lethargic consult your physician. Ascoril LS does not has any significant side effects.
Combiflam is a very safe drug without any significant side effects if given in prescribed dosage.

The prescription is for Ascoril LS. However there is no probiotics prescribed. So could you please suggest a probiotic and its dosage?

I'd also like to know what food I can give him. He is having low appetite and we are giving him only cow's milk, Cerelac (stage 5 cereals&fruits) and oats. Is this fine? Should I include or exclude any specific diet? Thanks.

You can give bifilac XXXXXXX or enterogermina minibottle.Both these are probiotics.
Give it once a day for 5 days. Continue with the same feed. Nothing specific is needed to exclude or include in the diet. He can have whatever he feels like eating.

Dear Dr, Latest update: My kid vomitted 4 times in 2 days till yesterday night after badly coughing. The vomit contained sputum also along with food. The cough is so bad that there seems to be so much congestion from the sound. He has constant running nose, but temperature has subsided.

It is the 3rd day of the antibiotic course and we started probiotic as you suggested yesterday. He is moving around the house and not bed ridden as such. But he looks weak, his eyes seem dull and gloomy and he cries for small things.

Should we run a test for Dengue? Should we worry about him getting pneumonia due to this bad cough? In 18 months, this is the first time he seems to be this uncomfortable (mainly due to bad cough). Thanks.

I completely understand your concern.
Chances of pneumonia is very less as fever has subsided and there is no breathing problem.Severe cough followed by vomiting is most likely due to upper respiratory tract infection.

As cough is severe enough to cause distress and is sometimes associated with vomiting I would like to give some cough suppressant like dextromethorphan(like Ascoril D) to get the relief.I would also suggest you to give combination of antiallergic(chlorpheniramine) with decongestant(phenylephrine for running nose)like Timinic or Maxtra for early recovery.
All these are prescription medication so please get it prescribed by a physician.
The clinical symptoms of dengue are high grade persistent fever,vomiting,pain abdomen,rashes,headache and body ache which are not here,so I don't think Dengue test is needed.
It may take about 7-10 days for resolution of cough.
If you find any respiratory distress,lethargy,excessive irritability or any other significant symptoms please consult your pediatrician for evaluation and management accordingly.
