need assessment for drug free management
Detailed Answer:
Dear
Thanks for contacting Ayurveda with your heakth concern.
1. Since in history it is mentioned that you are suffering from 'syndrome' and not 'disease' thus do let me know about other features associated with PCOS like: weight gain, unwanted facial hairs, impaired blood sugar & thyroid levels, irregular menses?
PS. as you are taking Metformin which is prescribed for above said features, do let me know about current values and features.
2. Is PCOS primary or secondary to some disease? (Opinion of Gynecologist)
3. Duration of starting the medicine and about your marital status?
4. Do let me know about your AGNI [Metabolism], since low digestive fire is held responsible for development of features of this SYNDROME [cause and effect]
5. Please revert back for further Management & prophylactic aspects.

It's been 10 months I have been diagnosed with this condition. I have consulted doctor because of my missed period, and the doctor based on the symptoms I have chin hair, weight gain and through blood test identified insulin resistance, excess androgens in blood, missing periods diagnosed with PCOS. My thyroid levels are normal. I am married and trying to conceive. My doctor prescribed me Metformin for insulin resistance. It is working good.My periods are regular now. I have been watching my diet and exercising, by which I reduced weight and feeling good. My doctor recommended to be on Metformin until I get conceived and to discontinue after first trimester of pregnancy. I am ok with this suggestion. But, what about afterwards? I do not want to be on western medicine lifelong for this condition. I am looking for an alternative way to manage it. Please let me know if you have any additional queries.

Thanks for reverting back.
1. Since blood values is/are normal and there is feeling of well being among you, the said drug is working in tandem with the body and its needs.
PS. since you are taking measures to conceive, don't go for possible do's and dont's regarding medicines, since patient suffering from high blood sugar/insulin resistance [especially female] have chances of Gestational Diabetes.
2. Since parameters are normal, thus dietary regime regarding PCOS can be initiated:
i. AVOID: refined carbohydrates (packed cereals,white bread-rice-flour,pasta, cakes ,biscuits,sugar, honey, fruit juice concentrates,’starch’,watermelon, cantaloupe, pineapple,potato,carrots,dried fruits,ice cream,chocolates, jellies, pizza), fatty food,curd,late night work,milk,mustard oil,cabbage,soybean
ii. TAKE: fresh green vegetables, whole grains, olives, whole legumes (peas, beans, lentils),brown rice, oat meal, barley, green gram, saindhav [rock salt] lavan, tandulodak [rice gruel],coconut water, vegetable soups.
3. since PCOS is due to low metabolism (digestive fire) thus medicine will work/ function by increasing the Metabolism by acting as:
. XXXXXXX [appetizers],
. pacahana [digestion],
. ushna [hot],
. tekshna [sharp],
. sukshma [subtle],
. lekhana drugs [anti obese and anti kapha/fat] and thus reducing the symptoms.
PS. kindly consult with an Expert Ayurveda Physician nearby you, who after careful assessment of your XXXXXXX [constitution] will develop best Natural Management of Herbs.
4. We along with Prophylactic Management give Panchkarma Procedure called 'Uttar Basti' to female patients [In Ayurveda we call it ARTVA VAHA SROTO DUSHTI [Blocked channels], along with certain yogic exercises like Simhasan, Vajrasan, Shalabhasan, Sarvangasan.
PS. If you are interested, you may contact us.
5. Regarding conception, in Ayurveda what is written and what we usually tell our patients [under guidance] after above said procedure/medicines is:
. Take menstrual blood [few drops] on clean white cloth and wait for few seconds
. Avoid using any soap/detergents and wash the cloth with plain warm water.
. If there is any stain left behind there is imbalance of hormonal functions and if there is no stain [the female is ready for conception]

I have been following the same diet tips you suggested. Dairy free, sugar free, simple carb free diet . I heard dairy is not good for PCOS, so I stopped taking dairy products(milk, curd, cheese ....).why is dairy bad for PCOS?
I will check for harmonal imbalance, and if harmonal imbalance exists, can I take shatavari to regulate harmones. If so, should it be taken life long and does it have any side effects?
You are spot on about the suggestion of do's and dont's in the stage of conceiving. Metformin is working in my current scenario, and I am going to use Metformin for successful pregnancy. Whatever questions I am asking are for after the pregnancy and delivery. I am not going to experiment with any new ones at this stage.
And how about triphala powder for detox? Can it be taken for long periods of time without side effects? This is also for after the pregnancy.
Clarifications and Explanation

Thanks for reverting back.
1. Dairy products are best avoided because of multiple facets:
. these contains 'sugar' i.e. carbohydrate which will cause rise in Insulin [physiological reaction of body] and as Insulin resistance is already there thus will produce similar symptoms of the same.
. Since women with PCOS have high level of androgens [present in your history], as such chances of developing ovarian cancer are more and it is studied that IGF-1 [insulin like growth factor] and galactose in milk increases the risk of cancer even further.
. high androgen level and Insulin resistance; both raises the androgen level thereby increasing Dihydro testosterone (DTH) a metabolite of testosterone which promotes oil production resulting in clogged pores and ACNE.
PS. You can opt for unsweetened coconut and raw dairy products [which are not pasteurized] after consulting with your physician.
2. Regarding hormonal profile check with LH:FSH ratio [leutinizing: follocle stimulating] hormone ratio. Shatavri is an immunomodulator, sexual tonic, promotes breast milk, menstrual regulator with digestive and anti oxidant actions, thus improves overall fertility with no toxic/side effects reported on long term use. [thus can be taken safely]
PS. it is the best source of plant derived estrogen.
3. Haritki and Vibhitak present in Triphla are contraindicated in pregnancy and not Amla and since you will be breast feeding afterwards so taking triphla powder may cause loose stool in your baby but also protect from infection and provides immunity to the baby [so only you are the best judge at that moment].
PS. You can start it once you are not lactating and baby has been put on weaning or other semi-solid/solid food.

Thank you for clarifying the question regarding milk. How about unpasteurized goat milk? Does it have the same effects?
Clinical Efficacy
Detailed Answer:
Dear
Thanks for reverting back.
1. Shatavari contains phyto estrogens [precursor of estrogen] which is useful as;
i. in female who have attained Menopause, with hysterectomy/oophrectomy [where there is natural low level of estrogen.
ii. In Menstruating women it help to regulate menstruation and ovulation.
iii. The estrogen does not produce any side effect which is the reason of calling it as 'Queen of Herbs' and also it is high in folic acid and helps prevent anemia
PS. phyto estrogens found in Shatavri 'binds' to estrogen receptors [estrogen regulator] and PRESERVE them from damage due to phyto estrogens present in processed products. [reason of using them in pregnancy].
If you are uncertain or if estrogen level come out high you may consider 'Maca' and/or 'Ashwagandha' as alternate.
2. Since pregnancy is the best time of one's life, so again I will not recommend any do's and dont's because of the following reasons
i. Since unpasteurized milk can harbor harmful bacteria which can prove fatal for growing fetus, thus always check with your health care provider before adjusting/changing/deleting any prescription.
ii. It is common practice to make cheddar/swiss cheese from unpasteurized milk and is consider safe.
