Brief Answer:
Buprenorphine transdermal patch

Detailed Answer:
Hi XXXXXXX
Methadone is a safe and effective long acting opiod analgesic that is useful in managing chronic pain.
Your wife can take maximum dose of methadone of 930 mg per day.
If the pain is not controlled by methadone then she can use Buprenorphine transdermal patch. It controls pain for 7 days continuously. It is available at strengths of of 5, 10 and 20ug/h
She can apply one patch to a dry, non-irritated, non-hairy area of her skin on the upper body such as your chest, upper back, or the outside of her arms. She has to leave it in place for seven days and then remove it and replace it with a new patch, but apply the replacement patch to a different area of her skin on the upper body.
I would recommend 20 ug/h patch for her.

Take care
Regards
DR De
