I have gone through your medical history in detail.
From the description,it seems to be a callus arising from the deep layer of the skin or the subcutaneous fascia.
This may be due to prolonged standing hours on a hard surface (concrete).
The skin below the 4th / 5th toes is the part which is subjected to maximum strain and pressure friction during standing hours.If the insoles of your shoes are hard,then it leads to such painful callus formation.
I suggest you to consult a dermatologist as soon as possible.
LASER treatment for painful callus has excellent results and you should talk to your doctor about this.
Take anti inflammatory tablets like ibuprofen,paracetamol till then.
Application of local creams will help till then.
